- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. THE SCHEDULE SERVICES AND PRICE/COSTS REQUIREMENTS Contractor shall perform preanalytic processing analysis, and result interpretation for clinical and anatomical pathology specimens as requested by Blanchfield Army Community Hospital, Fort Campbell, KY, hereinafter referred to as BACH. Services to be fur nished under this contract shall be ordered for results to be delivered to BACH in accordance with standards, clauses and provisions of this document. SCOPE OF WORK: Contractor shall provide laboratory services to include pickup and transport of specimens to its laboratory; preanalytic processing as defined in its laboratory users manual; analysis, reporting of analytic results, and consultation regardin g selection, collection, transportation and result interpretation. The contractor shall provide the following services: 1.Provide all necessary supplies, not limited to the following: ?Requisition forms;Specimen containers;Dry ice and appropriate container;Special instructions; Current list of tests with reference ranges and specimen requirements;Log books for samples sent out and results returned;Specimen carriers;All forms, including Chain of Custody forms;Special media or tubes for viral samples;All supplies for special tests (e.g., but not limited to, viral studies);Provide specimen pick up services as defined in the contract.3.Transport samples in such a manner as to ensure the inte grity of the specimen. 4.Contractor shall supply any special preservatives required for specimen preservation.5. Analyze samples.6.Routing test results shall be reported within 24 hours of specimen pick up. Critical Value test results shall be reported immediately. 7.Provide test report summaries by the 3rd workday of each month following the month in which service was delivered.8.Consult with Laboratory on test results by telephone as needed.9.Provide MTF Laboratories with a means of communication to permit immediat e inquiry regarding the status of pending tests.10.Billing and test report summaries shall begin the first day of the month and include the last day of the month.This solicitation must be quoted all or none.This is an important requirement for Blanchfield Army Community Hospital. The laboratory services are provided to MTF and its patients are an integral function in the daily operations of the hospital. All proposed dates/start-up time submitted must be realistic and valid dates that must be adhered to.W ithin 48 hours after award, contractor (s) shall notify the Contracting Officer that they have contacted the Medical Treatment Facility (MTF) and understand all requirements.REQUIREMENT IS FOR BASE YEAR, 01 Oct 04, with four one-year option/award term year s.1.Submit proposal for the following requirements at Blanchfield Army Community Hospital, Ft. Campbell KY, to Ms. Sebrena L. Hackley-Lane, SE Regional Contracting Office, Bldg 39706, 40th Street, Ft. Gordon GA 30905. Proposal is required no later than 31 Aug 05, 4:00 EST.2.PLEASE NOTE: Payment shall be in arrears NET 30. Please read the Performance Based Work Statement thoroughly and make sure you can provide properly qualified services by the performance start date. Start-up for performance as requi red/stated herein shall be no later that 01 Oct 04. Performance must begin 0n 01 Oct 04.
