Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotations are being requested and a written solicitation will not be issued using Simplified Acquisition Procedures under the test program for certain Commercial items found at FAR Part 13.5. This requirement is being issued as a Request for Quotation. Reference Purchase Request (PR) Number F13ACS42192800 on your RFQ. This document and the incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2001-16 Effective 01 Oct 2003 the DCN was updated 20031001. This requirement is issued as a 100% small business set-aside. The North American Industry Code System number is 422920 and the size standard is 500 employees. Description of requirements: 700 copies of ?Plan of Attack? by Bob Woodward IBSN#074325547X. Place of delivery will be Maxwell AFB and time of delivery will be no later then 30 Sep 04. This is a competitive best value acquisition utilizing simplified procedures authorized by FAR 13.5. A firm-fixed price contract will be awarded to the lowest acceptable price contractor that meets all requirements of this solicitation. The following Federal Acquisition Regulations (FAR) clauses apply to this acquisition and are incorporated by reference: FAR 52.212-4 (Oct. 2003), Contract Terms and Conditions ? Commercial Items and is addended to add FAR 52.204-4 (Aug 2000), Printed or copied double-sided on Requested Paper; and FAR 52.212-5 (June 2004), Contract Terms and Conditions Required To Implement Statutes Or Executive Orders ? The following provisions apply to this acquisition; FAR 52.212-1 (Jan. 2004), Instruction to Offerors- Commercial Items. Offerors must include a completed copy of the provisions at FAR 52.212-3 (May 2004), Offer Representations and Certifications- Commercial Items, with its Alternate I; and Defense FAR Supplement (DFARS) 252.212-7000 (DEC 1991), Offeror Representations and Certifications--Commercial Items.. The following DFARS clauses apply to this acquisition and are incorporated by reference: DFARS 252.204-7004 (NOV 2003), Required Central Contractor Registration (CCR), CCR can be obtained by accessing the Internet, http://www.ccr.gov or calling 1-888-227-2423; DFARS 252.212-7001 (AUG 1999), Contract Terms and Conditions Required to Implement Statutes or Executive Orders, and specifically the following clauses under paragraphs (a) and (b) are applicable; FAR 52.203-3 Gratuities; DFARS 252.232-7003 (DEC 1991), Electronic Submission of Payment Requests; DFARS 252.225-7002 (APR 2003), Qualifying Country Sources and Subcontractors; and DFAR 252.243-7002 (MAR 1998).
