- THIS IS A COMBINED SYNOPSIS SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR, SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. SOLICITATION NO. FA4621-04-Q-0546 IS A REQUEST FOR QUOTATION (RFQ). THE SOLICITATION DOCUMENT AND INCORPORATED PROVISIONS AND CLAUSES ARE THOSE IN EFFECT THROUGH FEDERAL ACQUISITION CIRCULAR FEDERAL ACQUISITION CIRCULAR (FAC) FAC 2001-24, EFFECTIVE 19 JUL 2004 CLASS DEVIATION 2004-00001. THE SOLICITATION IS 100% SMALL BUSINESS SET ASIDE. THE NAICS/SIC CODE IS 443220/5023 AND THE SMALL BUSINESS SIZE STANDARD IS 500 EMPLOYEES. THIS SOLICITATION IS FOR THE FOLLOWING: CLIN 0001: PURCHASE AND INSTALL CARPET AND TILE: ALL WORK SHALL BE DONE IN ACCORDANCE WITH ESTABLISHED INDUSTRY STANDARD. THE CONTRACTOR SHALL PROVIDE ALL PERSONNEL, EQUIPMENT, TOOLS, MATERIALS, SUPPLIES, SUPERVISION AND TRANSPORTATION NECESSARY TO REMOVE 1700 SQ YDS OF GLUED DOWN CARPET AND THEN INSTALL, TILE, COVE BASE AND TRANSITIONS STRIPS. THIS PROJECT REQUIRES THE INSTALLATION OF APPROXIMATELY 1500 SQ. YDS OF MOHAWK COMMERCIAL CARPET, TARENTON, STYLE NO.26, COLOR: 941 STONEHENGE (OR EQUAL); AND APPROXIMATELY 200 SQ. YDS OF GOVERNMENT OWNED CARPET AS DESIGNATED. THE CONTRACTOR WILL BE REQUIRED TO PICK UP GOVERNMENT PROVIDED CARPET AT HOLLOMAN AFB STORAGE SITE AND TRANSPORT TO THE WORK SITE. CONTRACTOR SHALL REMOVE APPROXIMATELY 4200 SQUARE FEET OF VINYL TILE AND STAIR THREAD FROM ALL AREAS. CONTRACTOR SHALL ALSO INSTALL FIELD CERAMIC TILE, MANUFACTURER: FLORIDA TILE INC, SIZE: 12?X12?X1/4?, STYLE: EXCAVARA, NATURA SERIES, COLOR: 9916 FOSSIL, (OR EQUAL) AND INSTALL ACCENT CERAMIC TILE SAME MANUFACTURER, COLOR SHALL BE 9906 CHARCOAL (OR EQUAL). INSTALLATION OF NEW COVE BASE WILL BE APPROXIMATELY 5400 LINEAR FEET, MANUFACTURER: JOHNSONITE VINYL 4?, COLOR: 29 MOONROCK (OR EQUAL). THE CONTRACTOR SHALL DISPOSE OF ALL CARPET AND COVE BASE REMOVED AND ANY RUBBISH CREATED AS A RESULT OF THIS PROJECT. INSTALLATION SHALL BE PERFORMED IN A WORKMANLIKE MANNER AND IN FULL COMPLIANCE WITH MANUFACTURER?S RECOMMENDATIONS SO AS TO PRESERVE ALL WARRANTIES. FAR 52.212-1, INSTRUCTIONS TO OFFERORS COMMERCIAL ITEMS SUBMISSION OF OFFERS (JAN 2004) AND FAR 52.212-2 EVALUATION OF COMMERCIAL ITEMS (JAN 1999). SUBMIT SIGNED AND DATED OFFERS TO THE OFFICE SPECIFIED IN THIS SOLICITATION AT OR BEFORE THE TIME SPECIFIED IN THIS SOLICITATION. OFFERS MAY BE SUBMITTED BY LETTER, FAX OR EMAIL.
