- The Department of Veterans Affairs is issuing RFP # 101-39-04 as a Request for Proposal for commercial items. This is a combined synopsis/solicitation prepared in accordance with the format in Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document incorporates provisions and clauses in effect through FAC 01-24. This acquisition is unrestricted. The associated NAICS Code work shall be performed under is 541720. The Office of the Medical Inspector (OMI) has a requirement for survey consulting services at VA Nursing Homes. The requirement is defined in the attached Statement of Work. The Government intends to award a Firm-Fixed Price contract. The period of performance shall be six (6) months from date of award. The provisions at 52.212-1 Instructions to Offerors-Commercial Items, and 52.212-3 Offeror Representations and Certifications-Commercial Items apply to this solicitation. Offerors are responsible for returning a copy of the Representation and Certifications at FAR 52.212-3 with their proposal. A copy of the provision can be obtained at www.arnet.gov . Commercial items FAR clauses 52.212-4 Contract Terms and Conditions-Commercial Items, and 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders ? Commercial Items, apply to this acquisition. All Contractors are required to register in the Central Contractor Registration (CCR) database prior to being awarded a contract. Contractors are responsible for the accuracy of information added to CCR. To register go to http://www.ccr.gov/. Evaluation Criteria The objective of this procurement is to choose a single ?best value? proposal that provides trade-offs that are advantageous to the Government. The trade-off methodology was selected as the most advantageous method available to the Government because the subject is complex, lends itself to creative operations, and performance risk is considered to be high. Innovative approaches and solutions are encouraged. The following evaluation criteria shall be used to evaluate offers in order of descending order of preference: 1.Understanding the Problem 2.Knowledge and Experience of Proposed Personnel 3.Cost 4.Past Performance of Offeror Understanding of the Problem is the most significant criteria of all. Past Performance of Offeror is the least significant factor. If in evaluating proposals it is determined that an Offeror misrepresented its capabilities, it could be a significant factor in both the evaluation and the contracting officer's responsibility determination. Due Dates The due date for all questions concerning this RFP must be received by August 18, 2004 at 12:00 PM EST via email to: Lauren.Yates@mail.va.gov . All responses to questions will be responded to all offerors no later than 5:00 PM EST, August 23, 2004. Please format questions by: page, paragraph, or general. All questions must be submitted via email. No calls concerning this acquisition will be accepted.
