Description
The proposed contract action is for services which the Naval Medical Center San Diego intends to solicit for a Non-Personal services of an under the authority of FAR 6.302. Interested contractors/persons may identify their interest and capability to respond to the requirement or submit quotations. This notice of intent is not a request for competitive quotations. However, all quotes received by August 12, 2004, 3.00.PM., Pacific Standard time, will be considered by the Government. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-04-T-0053 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 541990, Size: 6.0. This procurement is a small purchase, small business set-aside solicitation. Clin 0001. Contractor shall provide On-site Non-personal services contract of an Opthalmic Technician, Period of Performance September 1, 2004 through August 31, 2005. QTY:2072 Unit of Issue: Hours $______ = $ ______. The duties to be performed shall be in accordance with the statement of work, (SOW). The services and duty hours will be performed at Naval Medical Center San Diego, Opthalmology Department, BMC/NTC, 34800 Bob Wilson Drive, San Diego, Ca 92134. STATEMENT OF WORK OPHTHALMIC TECHNICIAN I. The contractor shall serve as a COA/OT/COMT in support of the Corneal Refractive Surgery Programs for the NMCSD or Naval Training Center, San Diego. You shall provide 80 hours of service every two weeks. Services shall be provided for an 8.5 (to include .5 hours uncompensated for lunch) or 9 hour (to include 1 hour uncompensated for lunch) period between the hours of 0700 and 1800. Services shall typically be provided Monday through Friday, however, Saturday morning services may be required. Specific hours will be scheduled by the Commanding Officer. If the contractor is required to stay past the normal work hours, the contractor will either be compensated with regular pay (no overtime) or compensatory time off at a later date to be mutually agreed upon. Should services be required at one of the branch medical clinics of the medical center, the health care worker shall be given 1 to 2 weeks advance notice prior to such a change in location. Such changes will be temporary. The health care worker shall arrive for each scheduled shift in a well-rested condition and shall have had at least six hours of rest from all other medical duties. You shall NOT accrue leave hours worked. Your services shall not be required on the following federally established paid holidays: New Year's Day, Martin Luther King's Birthday, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Thanksgiving Day, and Christmas Day. You shall not be compensated by the Government for periods of planned absence. 1 II. Statement of Work 1. The use of "Commanding Officer" means Commanding Officer, or designated representative, e.g., Technical Liaison or Department Head. 2. Health care workers providing services under this contract shall be rendering non- personal services to the Government and shall not be subject to supervision and control by Government personnel. . 3. Duties and Responsibilities. You shall be responsible for delivery of treatment within the personnel and equipment capabilities of the MTF, provision of mandated medical surveillances and preventive services, and the quality and timeliness of treatment records and reports required to document procedures performed and care provided. 4. Administrative and Training Functions: You shall provide training and/or direction as applicable to supporting Government employees (i.e. hospital corpsman, technicians and students) assigned to you during the performance of clinical procedures. You shall perform limited administrative duties, which include the following: maintaining statistical records of clinical workload, completing Ambulatory Data System sheets to account for care provided, participating in education programs and research, and participating in clinical staff quality assurance functions and Process Action Teams, as prescribed by the Commanding Officer n the chain of command. Additionally, you shall: a. Participate in the provision of monthly in-service training to non-health care-practitioner members of the clinical and administrative staff on subjects related to eye care if requested. b. Participate in weekly and monthly meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist. Should a meeting occur outside of regular working hours, the health care worker shall be required to read and initial the minutes of the meeting. c. Attend that portion of the command orientation classes that provides an overview of command resources and emergency response. d. Attend all annual retraining classes required by this command. e. Family Advocacy. Participate in the implementation of the Clinic’s Family Advocacy Program as directed. f. Obtain certification in American Heart Association Basic Life Support (BLS) for Healthcare Providers; American Heart Association Healthcare Provider Course; American Red Cross CPR (Cardio Pulmonary Resuscitation) for the Professional Rescuer; or equivalent. This training and certification will be provided by the Navy. Also, maintain current health requirements required by the command (ie. Yearly PPD). 5.Clinical Duties. a. Perform basic visual acuity tests b. Measure and record distance and near vision; perform color testing and pinhole vision c. Measure and record low contrast visual acuity d. Perform Contrast Sensitivity testing e. Perform Lensometry f. Perform Autorefraction and Auto keratometery g. Perform and interpret refraction with both automated and manual phoropters h. Perform basic ocular motility tests i. Perform visual field testing – automated and manual j. Perform Ophthalmic photography k. Perform and Assess Corneal topography l. Perform Wavefront analysis m. Assist during refractive surgery n. Perform Glare Testing o. Teach Contact Lens care p. Perform Goldman Tonometry q. Perform Mesoptometry r. Perform Pupilometry s. Perform Ultrasonic Corneal Pachymetry t. Perform Orbscan u. Perform Night Driving Simulator testing v. Direct the flow of patients: obtain medical and personal data for patients for conditions excluding ocular examination; arrange for laboratory and other tests, ophthalmic photography, outpatient studies and patient admissions and surgery as requested by the physician. Alleviate patient and family concerns by explaining diagnostic procedures and referring specific questions to the doctor. w. Screen walk-in patients, emergency patients and telephone calls from patients and referring physicians to determine appropriate physician and priority of appointments. x. Perform inspection and preparation of instruments, materials, and equipment to insure that they are clean and in proper working condition. y. Sterilize and set up instruments for surgical procedures; assist with minor office surgery, laser treatments and operating room surgery as required. Break down surgical OR when completed at end of day. z. Perform inventory of assigned examination rooms to insure adequate supplies are in stock. This shall be coordinated with the supply officer of the CRS Program to ensure all supply requests are met. aa. Adhere to infection control guidelines and practice universal precautions. bb. Be responsible for the safe and effective operation of equipment used in patient care and contributing to a safe working environment. This shall include safe practices of emergency procedures. Proper handling of hazardous materials and maintenance of physical security. 6. Personnel Qualifications a. You are required to possess the following minimum levels of professional and technical experience. The specialized experience included as part of the required qualifications shall have been obtained in the fields of endeavor indicated by the following applicable labor categories: Certified Ophthalmic Technician b. Possess at least 3 years experience within the preceding 5 years as a COT, performing functions such as the following: visual acuity, refraction, color vision testing, automated visual fields, topography, applanation tonometry, slit lamp photography, and assisting in refractive surgical procedures. c. Possess a working knowledge of ocular physiology, anatomy, pathology, optics, refraction and contact lens principles. d. Possess an extensive knowledge of ophthalmic procedures, instrumentation, and techniques as related to the field of eye care. e. Possess an extensive knowledge of refractive surgery to include familiarity with the Excimer laser, PRK and LASIK, patient selection, and care of refractive surgery patients. f. Possess certification as a Certified Ophthalmic Technician (COT) as determined by the Joint Commission on Allied Health Personnel. g. Represent an acceptable malpractice risk to the Navy. h. Have U.S. employment eligibility per Attachment III. No alien shall be allowed to perform services under this contract if in violation of the Immigration Laws of the United States. i. Ability to communicate orally and in writing using grammatically correct English .. j. Knowledge of grammar, spelling, capitalization, and punctuation to process a variety of correspondence accurately. k. Ability to compose technical and non-technical correspondence from handwritten notes, recordings, etc. l. Knowledge of Navy Correspondence formats, forms, command correspondence policies and military (Navy/Marine) terminology required. m. Basic knowledge of Navy and Marine Corps rank and military customs and courtesy.. n. Knowledge of standard office equipment such as computers, copiers, fax machines, etc. to perform routine clerical and administrative functions. o. Knowledge of PCs and software such as word processors, graphics, spreadsheets, database management systems and communications; specific familiarity with MS Word, Excel, PowerPoint, Access and Outlook.. p. Specific skills in computer applications include the ability to input data to obtain reports, create forms, compose correspondence, create charts, graphs, messages, letters, etc. in Microsoft Office, Word, Power Point, Excel, Access, Outlook and various other programs developed for the Navy’s use. q. Knowledge of Privacy Act and HIPPA regulations pertaining to the confidentiality of Health records, medical information contained therein, and the security requirements of such documents and information .. r. Provide three letters of recommendation attesting to experience and capability in the conduct of clinical ophthalmic/vision research. The letters should be from practicing eye care specialists or research managers. This requirement may be waived for an applicant who has previously worked at the Navy Refractive Surgery Center NMCSD. Accordingly, the contractor shall furnish such resumes and all professional requirements as stated in the Statement of Work (SOW) to the Contract Specialist along with their quote.
