Description

The Oklahoma City VA Medical Center has a requirement for utilization of a pre-established community-based clinic with physicians and personnel for primary care and mental health care. Routine and followup services to include normal x-ray, laboratory and pharmaceutical services. Clinic must be located within a 30-mile radius from the Center of Wichita Falls, Texas and be located within our primary service area in the State of Texas and must be ready for operation at time of award. Physicians and technicians operating under the resulitng contract must meet all evaluation criteria and other professional certificatoins required to operate in such a clinic. Clinic profiles will be detrmined by the VA Medical Center Director as workload demands. The resulting contract covers up to a ceiling or maximum of 2500 primary care patients and 400 mental health patients. Please note that these are estimates only and that enrollment for these exact amounts are not guaranteed. Solicitation issue date shall be on or about July 27, 2004 with a response due date of August 27, 2004.
