- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation documents incorporated provisions and clauses are those in effect through Federal Acquisition Regulation Circular 2001-24. This procurement is set aside 100% for small business. The NAICS is 423450 with a business size standard 500 employees. A firm-fixed price contract is anticipated. Products listed herein are products manufactured by Motion Lab Systems, Inc and in accordance with FAR 52.211-6 Brand Name or Equal and VAAR 852.211-77 Brand Name or Equal are intended to be descriptive but not restrictive, and are provided to indicate the quality and characteristics of products that will be satisfactory. The South Texas Veterans Health Care System (STVHCS), Audie L. Murphy Division, 7400 Merton Minter Blvd., San Antonio, TX 78229 has a requirement to purchase a Diagnostic Electromyograph (EMG) System, 16 Channel of EMG (bandwidth range 20Hz to 2000Hz), 4 additional research channels, and 8 event channels, part number MA-300-16. System shall be complete with the following items: Slim Desktop Unit for MA300 EMG system, part number MA-300-DTU; 16 Channel EMG Backpack Unit; part number MA-300-BPU; Double differential, reusable, surface EMG pre-amplifier, x20 gain; part number AMP/MA311; Coaxial EMG signal cable. part number BPU/MA133; Shielded analog signal cable, part number MA130; 1mm thick by 25mm diameter membrane contact switch, part number MA153; Foot-switch cable assembly, part number MA335; Foot-switch cords, part number MA136; Bead marker set, part number MA321; 32? reusable ground electrode, part number AMP/MA391; Coban Latex free self-adherent wrap, part number AMP/MA395/CBN; Latex free, neoprene child vest, part number MA142; Latex free, neoprene adult vest, part number MA143; Pack of 3 pediatric monitoring electrodes, part number AMP/MA390; and Hospital grade power line cord, part number MA300/PLC/US. Quotations shall include all warranty information, technical data, specifications and shop drawings, etc. Quotations shall be submitted F.O.B. destination. Murphy Division, intends to award a firm fixed price contract for one (1) Versa pulse Power suite 100W, part number 0638-803-01, Biomedical Training ?Tuition Only, four (4) Power suite Spectacles For Safety, part number 0638-429-01 and one (1) day of Physician Holmium Ablation Training. The Power suite Laser is used for ablation vaporization, excision, incision, and immediate hemostasis of soft tissue through a scope or open access. This announcement constitutes the only solicitation. Request For Quote (RFQ) 671-69Q-04 is being requested and a written solicitation will not be issued. Quotations shall include all warranty information, technical data, specifications and shop drawings. Quotations shall be submitted F.O.B. destination.
