Description
This is an Indefinite Delivery Indefinite Quantity, Firm Fixed price contract. Pricing should be established L-Code prices applicable to the geographic area to furnish artificial limbs and related services to veterans that require them and are eligible for assistance to Department of Veterans Affairs Veterans, 5001 N. Piedras Street, El Paso, Texas 79930. THERE ARE NO FILES TO DOWNLOAD. THIS SOLICITATION IS SUBJECT TO AVAILABILITY OF FISCAL YEAR 2005 FUNDS. HOWEVER, FUNDS ARE PROGRAMMED TO BE AVAILABLE FOR FISCAL YEAR 2005 CONTRACT PERFORMANCE. THIS IS A SOLICITATION ANNOUNCEMENT FOR REQUEST FOR PROPOSALS FROM QUALIFIED FIRMS INTERESTED IN PROVIDING THEIR OFFERS FOR FABRICATION OF ARTIFICIAL LIMBS. The Contractor shall fabricate all artificial limbs in strict conformance to the prosthetic prescription that has been provided by the VA Amputee Clinic Team. Contractors shall not add or substitute components or alter the beneficiary's limb prescription in any way without prior approval and written authority from the VA Amputee Clinic Team Prosthetic Representative or designee. All contractors shall have a full-time American Board Certified (ABC), or Board for Orthotist-Prosthetist Certification (BOC), or VA Qualified (VAQP) Prosthetist on staff at each of their locations in good standing. A waiver for this requirement may be granted on a case-by-case basis for branch offices only in the event that VA determines this requirement would place an undue burden on the veteran. The following evaluation factors will be used to evaluate the offers: (1) Price, (2) Technical, (3) Past performance. Competition is restricted to the local, commuting area for the convenience of the veterans. It is anticipated that at least two contracts will be awarded to ensure an adequate number of providers for the veterans. Firms interested and having the capabilities and qualifications to perform this work must request a solicitation from the following address: ATTN: Cathy Fernandez, Contracting Officer, 5001 N. Piedras, El Paso, Texas 79930. Proposal submittals are due at 4:00 pm, local mountain time, 30 days from issuance of the solicitation. The solicitation is expected to be issued on or before August 30, 2004.
