Description
The Program Support Center has a requirement for the Office on Womens Health (OWH) to establish rural/frontier womens health coordination centers. The purpose of this requirement is to establish coordinating centers to identify, coordinate, and leverage a network of existing resources to provide a full range of services to women and their families living in rural/frontier communities. The function of a coordinating center will be to provide culturally and geographically appropriate technical assistance for the development of a seamless system of health care that encompasses medical, dental, mental health, social services, and other enabling and support services with special emphasis on transportation needs. New community resources will not be provided under this contract. Existing community resources must be used in this effort. Successful offerors must be respected health care entities in the community that are able to adequately provide technical assistance and coordination for a complex and integrated multi-component range of services that encompass clinical outreach, leadership/mentoring, professional education and research. Successful offerors must be able to address all of these components on a large scale while partnering and networking in an integrated fashion to a very diverse group of persons and organizations in rural and frontier populations, hopefully to include the elderly, Native Americans, persons from other racial and ethnic groups. Successful offerors must have excellent and extensive coordination skills and be able to maximize enabling and social services in a community. The period of performance will be a one year base, with three one year option periods. This is a multiple award, cost-plus-fixed-fee requirement, with up to three awards anticipated. Solicitation number 04T004038 will be available on or about August 4, 2004, with a tentative closing date of September 2, 2004. The North American Industrial Classification System (NAICS) Code is 541990 and the business size standard is $6 million. The solicitation and any amendments, as well responses to questions shall be posted on the FedBizOpps web-page for downloading. No telephone or faxed requests for the solicitation will be accepted.
