- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number, H92244-04-Q-0071 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24 effective 18 June 2004. This procurement is UNRESTRICTED. All responses from responsible sources will be fully considered. This procurement has a rating of DO. NAIC Code 332912 size standards 500 employees or under. This synopsis/solicitation is for COMMERCIAL ITEMS. CLIN 0001 Support Services to Retrofit Micronel Blower Hoses, Quantity 700 each. Services to include removal of existing hose fittings, installation of High Strength Flouro (HSF) Polymer, and installation of swivel fittings on mask end of hose. Itemized breakdown of services shall include the following: a) Removal of existing hose fittings, b) Installation of High Strength Flouro (HSF) Polymer; c) Installation of swivel fitting on mask end of hose; d) Reinstallation of original blower end hose coupler fitting, serve and heat shrink both ends of the hose, and vacuum test hose upon completion of work. Government Furnished Equipment: 1) High Strength Flouro (HSF) Polymer, Quantity 700 each; 2) Swivel Fitting, Quantity 700 each; 3) High Strength Fluoro (HSF) Polymer, Quantity 700 each. Deliverables: Retrofitted Micronel Blower Hoses, Quantity 700 each to be delivered upon completion of contract. The provision at 52.212-1 Instructions to Offerors ? Commercial Items (Jan 2004) applies to this acquisition. The provision at FAR 52.212-2 Evaluation ? Commercial Items (Jan 1999) applies. The provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Items (May 2004) applies. The offeror must include a completed copy of this provision with their proposal. FAR 52.212-4 Contract Terms and Conditions-Commercial Items (Oct 2003) applies. FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (June 2004) applies. FAR 52.203-6 Restrictions on Subcontractor Sales to the Government (Jul 1995) applies. FAR 52.232-33 Payment by Electronic Funds Transfer-Central Contractor Registration (Oct 2003) applies. FAR 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items (Jun 2004) applies. DFAR 252.225-7001 Buy American Act and Balance of Payments Program (Apr 2003) applies. Quotations shall include the vendor?s cage code, Dun and Bradstreet Number (DUN), Tax Identification Number and Delivery Date. The prospective offeror must be registered with the Central Contractor Registration (CCR) prior to award.
