- This is a combined synopsis/solicitation. This requirement is for Hospital Linen/Laundry Services. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The solicitation number is FA4621-04-T-0006. The solicitation is issued as a Request for Quote (RFQ). This solicitation is reserved for small businesses; however, the Government will accept proposals from all businesses. In the event of insufficient small business participation, award will be made on unrestricted bases. The requirement is for a contractor to furnish all personnel, equipment, transportation, and supplies to provide laundry and dry cleaning services including pick up and delivery at the Medical Healthcare Facility, McConnell Air Force Base, KS. Performance shall be in accordance with the scope of a Statement of Work (SOW) and other specifications or provisions of the contract. Government-Furnished Property and Services: 1) Bags for contaminated items 2) Scales (for weighing laundry) will remain at the Government facility and will be maintained by the Government. Contractor-Furnished Items: 1) Delivery vehicle 2) All packaging and marking material 3) Laundry Cart 4) Hangers 5) Delivery tickets 6) Any other supplies, materials, and equipment necessary to perform the services required by this contract 7) A facility equipped and ventilated so as to prevent dissemination of contaminants. Estimated total weight for the annual period of performance is 22,065 pounds. The period of performance will be from October 1, 2004 through September 30, 2005 with options as follows: Option 1: October 1, 2005 through September 30, 2006. Option 2: October 1, 2006 through September 30, 2007. Option 3: October 1, 2007 through September 30, 2008. Option 4: October 1, 2008 through September 30, 2009. The NAICS code for this acquisition is 812331 (Size $12 million) and the SIC code is 7211. FAR Provisions 52.212-1 Instructions To Offerors-Commercial Items; 52.212-2 Evaluation-Commercial Items; 52.212-4 Contract Terms and Conditions-Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items; and DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items shall apply to this solicitation. The contractor is obligated to follow those publications coded advisory to the extent necessary to accomplish requirements of the SOW. Statement of Work can be furnished by fax or email upon request. Please submit with your quote a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items. All quotes will be evaluated on price. The Government will award a contract to the responsible offeror whose offer is conforming to the solicitation and will be most advantageous to the Government. Contractors submitting quotes must be enrolled in the Central Contractor Registration Program (CCR).
