- The VA National Acquisition Center will issue RFP 797-NC-04-0013 for the Department of Veterans Affairs (VA) and Department of Defense (DoD) requirements for Fluocinonide 0.05% Cream, Gel, Ointment and Solution. Fluocinonide 0.05% cream has an estimated annual usage of 58,130 tubes of 15gm, 13,509 tubes of 30gm and 104,786 tubes of 60gm. Fluocinonide 0.05% gel has an estimated annual usage of 10,365 tubes of 15gm and 7,131 tubes of 60gm. Fluocinonide 0.05% ointment has an estimated annual usage of 16,241 tubes of 15gm, 10,907 tubes of 30gm and 64,691 tubes of 60gm. Fluocinonide 0.05% solution has an estimated annual usage of 2,683 tubes of 20gm and 16,235 tubes of 60gm. The estimated quantities listed above are for VA and DoD only. The estimated quantities for Indian Health Service (IHS) and Bureau of Prison (BoP) will be provided once the solicitation is issued. The contract period will be for one year plus four one-year option periods. The contract items will be distributed through the VA and DoD Pharmaceutical Prime Vendors (PPV). If the offeror is not a manufacturer, a signed agreement between the offeror and the manufacturer must be submitted with the offer and remain in effect for the duration of the contract period including the option years. If this agreement is not provided, the offeror will be considered non-compliant and removed from consideration. Offeror_s will be required to propose a price for each line item listed in the solicitation to be considered for award. Offeror_s must state the exact name (name that appears on label) by which the drug will be supplied and have a unique NDC number. Offeror_s must provide bar code labeling which includes at a minimum the NDC number. Additionally, offeror_s are encouraged to provide the expiration date and lot number in the bar code. All tubes must be individually packaged in boxes. The solicitation will be available for downloading, on or about the issue date of June 29, 2004 from the VA Office of Acquisition and Material Management_s (O&AMM) internet web site @ http://www.va.gov/o&amm/busopp/sols.htm. Offers will be due by 4:00 pm Central Standard Time on July 22, 2004.
