Description

Contractor to provide Follow-up on accounts aged over 120+ days. scope of work: 1. Contractor shall perform off-site follow-up service on claims aged over 120+ days. 2. The Contractor shall provide four off-site follow-up/ Collectors to assist Chinle Voucher Examiners. 3. The Assignment is confined solely to follow-up on claims. No billing will be done by Contractors. 4. The Contractor will be given access to the third party and accounts receivable package. The package will be utilized remotely. 5. Contractor will provide daily reports on all rebills and accounts that need to be closed. 6. The Contractor shall key in results on follow-up on each account into the accounts receivable message file. 7. The Contractor shall notify the project Officer, Chinle Hospital, within 24 hours of the event of any changes and delay in services having a direct impact to the project. 8. The proposed fee shall include the all associated costs. i.e. personnel, supplies, materials, housing, per diem, and transportation to and job site. 9. A progress report shall be presented to the project officer upon request and completion of the project. 10. The hourly rates shall be an all inclusive rate of all associated costs, I.E., Housing, Labor, Transporation, meals, rental car, materials, equipment, facilities, administrative costs, overhead, profit and other costs related to the pro- ject. Services to be provided July 23, 2004 through September 30, 2004.
