- The United States Government, represented by the U.S. Agency for International Development is hereby amending Solicitation No. 519-04-Q-005 to answer questions raised by potential vendors and provide clarification on some of the requirements of the solicitation: Question No. 1: Item No. 2 requests a ?Two Step Ladder?, does this mean that we need to include a separate ladder in our quote or can we quote a table which already includes a one step stool . Answer: It is fine to quote a table which already includes a one step stool. Question No. 2: Item No. 3 requests ?Stainless steel headboard?, are you referring to a stainless steel headboard or does it refer to the back support stainless steel portion of the bed. Answer: It is the back support section of a stainless steel bed. Question No. 3: For the Neonatal Laryngoscopes, (Item No. 12), do you need Fiber Optic (lamp is located in the handle and shine through light pipe in handle) or Conventional (lamp is located on the blade)? Answer: Please quote the Fiber Optic option Question No. 4: Item No. 14 requests Neonatal Meconium Aspirator, we would like you to clarify if you are requesting an aspirator with built-in suction system or some special cannula, please clarify. Answer: Yes, we need a built-in suction system. Question No. 5: Item 10 requests ?Doppler Fetal Ultrasound?, and as part of the characteristics you request a ?one piece?. U.S. manufacturers designs a type where the transducer is connected to the equipment through a cable, please clarify if we can quote this type. Answer:Yes, that type will be acceptable. Question No. 6: Can we quote refurbished equipment or any of the items requested? Answer: The equipment supplied under this solicitation has to be brand new. Question No. 7: Should we quote FOB shipment to Port of Acajutla? Answer: Shipment for the equipment shall be FOB Aduana Central San Bartolo, the Ministry of Health will clear the equipment from Customs. For the items where installation and training is requested, please include those costs separately from the shipping costs but as part of the item cost. Please note that the perdiem rate for San Salvador is $163.00. Question No. 8: Is it correct that payment will be effected after final acceptance, and no payment before that? Answer: Documentation for payment requirement is modified to read: 50% of the total amount will be processed to the supplier upon receipt and acceptance of the equipment at Aduana Central San Bartolo. 50% shall be processed upon installation and training completion. On-site Inspection requirement is modified to read: As a requisite for final acceptance by USAID/El Salvador of the equipment, the Ministry of health will inspect the equipment in the presence of the USAID?s representative and the Contractor?s representative at Aduana Central San Bartolo no later than 15 calendar days after receipt of the equipment, prior to acceptance by the Office of Health of USAID/El Salvador. Question No. 9: Can we fax you the quotation, or do we need to mail it in? Or can we e-mail it to you? Answer: Your quotation can be faxed or send by mail, but it cannot be e-mailed as it is sensitive but unclassified information (SBU). Question No. 10: If an item requested in not available from a U.S. manufacturer can we propose and alternative. Answer: Offerors can propose equal or equivalent products. Note: All quotations must include Contractor?s DUNS Number, TIN Number (Tax Identification Number) and must register in CCR (Central Contractor Registration)and a certification that the equipment quotes is made (assembled) in the United States of America. EXCEPT AS EXPRESSLY HEREIN MODIFIED ALL OTHER TERMS AND CONDITIONS OF THIS SOLICITATION REMAIN UNCHANGED NOTE: THIS NOTICE WAS NOT POSTED TO WWW.FEDBIZOPPS.GOV ON THE DATE INDICATED IN THE NOTICE ITSELF (14-JUL-2004).
