Description
The contractor shall provide either a digital Phillips or Siemens Mobile Cardiac Catheterization Lab on site in accordance with the terms and conditions of the contract for a period of 30 plus days during the construction period of a permanent lab buildout in the Phoenix VAMC as needed for continuation of medical services. Period of performance to commence October 2, 2004. The contractor will provide three consecutive days of applications training on site at Phoenix VAMC. The mobile cardiac catheterization lab will be located at the Phoenix VAMC. The unit shall have the ability to perform wide range of Cardiac Catheterization Lab exams. The contractor shall provide any information to assist in VA planning on the following items: specifications for ?pad? construction, proper site location, electrical requirements, telephone, water, data links, etc. Contractor shall provide all emergency service to include parts and labor, and preventive maintenance as recommended by the original equipment manufacturer (OEM) specifications after hours (4:30 PM or weekends). The mobile system shall be capable of interfacing with the VA VISTA and CPRS computer connections.Unit must have stretcher/bed lift accessibility. Unit shall have an intercom system between the control room and scanning room. A music system with and assorted music shall be included. RFQ 644-02-05 is anticipated to be solicited by August 1, 2004 with offers due August 23, 2004 and will be solicited under the Commercial Item requirements of FAR Part 12.
