- Description
- The Customs and Border Protection, McAllen Sector (McAllen, TX)is requesting a firm-fixed price quotation for physical exams, for employees over the age of 40 (forty). This is a combined synopsis/solicitation for commerical items prepared in accordance with format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. One contract is anticipated to be awarded as a result of this synopsis. The term of this contract is the day of the award of this contract until all physicals are complete but not later than September 31, 2004. The contractor shall provide personnel, materials, services, facilities and any other items and services not Government furnished necessary to conduct Physical Examinations for employees over the age of 40 as defined in this work statement, for the U.S. Border Patrol, McAllen Sector. The contractor shall perform to the standards and specifications in the proposed contract. The contractor shall provide Physical Examinations during the hours of 8:00am through 4:00pm, Monday through Friday, excluding Federal Holidays. The contractor shall provide the workforce possessing the skills, knowledge, and training to satisfactorily perform the services required by the contract. The contractor?s workforce shall be board eligible or certified by their appropriate occupational health or medicine discipline. A copy of certification shall be available in the office of the physical for review by any ordering agency. The contractor shall submit a Medical Liability Insurance Certificate to the Contracting Officer prior to the contract start date. DEFIITIONS DHS ? Department of Homeland Security USCBP - United States Customs & Border Protection CONTRACTOR ? the entity that provides the services described in this performance work statement. FAR ? Federal Acquisition Regulation, Chapter 1, 48 CFR (Code of Federal Regulation) HSAM ? Homeland Security Acquisition Manual, Chapter 3000 FEDERAL HOLIDAYS ? Holidays in each calendar year are identified as follows: New Year?s Day, January 1, Martin Luther King?s Birthday, the 3rd Monday in January, Washington?s Birthday, the 3rd Monday in February, Memorial Day, the last Monday in May, Independence Day, July 4, Labor Day, the 1st Monday in September, Columbus Day, the 2nd Monday in October, Veteran?s Day, November 11, Thanksgiving Day, the 4th Thursday in November Christmas Day, December 25. GOVERNMENT FURNISHED: The Government will provide the Employee Advisement Letter/form. The contractor shall furnish all property and services not specifically identified in GOVERNMENT FURNISHED above, but which are necessary to perform the requirements of this contract. The contractor?s facilities shall be located in or around a 100-mile radius of McAllen, TX, Harlingen TX, and Corpus Christi, TX. The performance of the services will occur at the contractor?s facility. SPECIFIC TASKS PHYSICAL EXAMINATIONS: The contractor shall provide Physical Examinations for employees over the age of 40. To accomplish this service, the contractor shall perform the following tasks. Physical Examinations shall include: Health and Occupational History FOH Profile ? 26 panel blood chemistry ? Complete blood count ? Urinalysis Vision Test Audiometric Testing ? Audiogram Tonometry for Glaucoma Resting Electrocardiogram Tuberculin Skin Test Tetanus/Diphtheria booster, if needed Test for occult blood in stool Physician evaluation of Major Body Systems Female Cancer Screen ? Pap Smear and Breast Exam Male Cancer Screen ? digital exam of prostate Physician review of all tests and referral if necessary Chest X-ray Sigmoidoscope (flexible) screen for colon cancer PSA test for prostate cancer Cardiovascular Fitness Profile shall include: Cardiac Laboratory Profile (glucose, triglycerides, cholesterol, HDL, LDL, Cholesterol/HDL ratio, risk factor). ECG monitored cycle or treadmill exercise test if the Cardiac Profile warrants additional testing. Physician shall provide the following information to each employee tested: Photocopy of all forms, tests, exams and their results, An employee Advisement Letter explaining medically significant data and any abnormal findings and providing recommendations for addressing these concerns (see attachment 1). For all exams performed, physician shall provide post-exam counseling only, with no formal follow-up communication other than the forms described in paragraph 5.1.3., above. If an imminent danger condition is found during the physical examination, the physician or nurse shall notify the ordering agency and patient by telephone or telegram followed by the information required in paragraph 5.1.3, above. Once the contractor has furnished copies of the forms mentioned in paragraph 5.1.3, above to the patient(s) a medical record is no longer required for purposes of this contract. Itemized invoices/bills will be submitted. The government will only pay for administered tests. The Contractor will schedule physicals by giving the employee at least 3 days notice of when and where to appear along with instructions about fasting, etc. for the laboratory tests. All exams shall be completed no later than September 30, 2004, and each employee shall have received written evaluation of his/her physical exam 30 days after completion of the physical exam. The Contractor awarded the contract must obtain a Data Universal Number (DUNS), and register in the Central Contract Registration System (CCR). The provisions of FAR 52.212-1, Instructions to Offerors-Commercial; FAR 52.212-2, Evaluation-Commercial Items; FAR 52.212-4, Contract Terms and Conditions-Commercial Items; and, FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, apply to this acquisition. Under the provision of FAR 52.212-5, the Walsh-Healy Public Contracts Act is incorporated by reference the stipulations required by the Act pertaining to such matters as minimum wages, maximum hours, child labor, convict labor, and safe and sanitary working conditions. FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era. FAR 52.222-36, Affirmative Action for Handicapped Workers. FAR 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era. FAR 52.222-3, Buy American Act-Supplies. The OFFEROR shall include the firm's DUNS (Data Universal Numbering System) number and as part of their offer. The Government will award a contract resulting from this solicitation to the responsible offeror whose quote, conforming to the RFQ, is most favorable to the Government with price and other factors considered. The following factors shall be used to evaluate offerors' technical capability. Evaluation will be based on price, and past performance, with award to be made to the lowest technically acceptable responsive responsible offeror. PAST PERFORMANCE: Each offeror shall describe their past performance on similar contracts held within the last three years which are of a similar scope, magnitude and complexity to that which is detailed in this RFQ. Past performance shall be evaluated in the following manner: The Government will assess the offeror's ability to perform, including the offeror's likelihood of achieving success in meeting the contract requirements. The offerors are to provide a narrative discussion of similar projects of a similar size and dollar value, which have been successfully completed in the past three years. A description of the progress of the projects currently in process should be included as well. Contracts listed may include those entered into with the Federal Government, agencies of state and local governments and commercial customers. Past Performance will be assessed and assigned a narrative rating in the evaluation. The Government will evaluate each offeror's past performance based on the information submitted by the offer, as well as information that the Government collects by itself. Offerors will be given credit for good past performance and lose credit for poor past performance, and neither receive nor lose credit for no past performance data. As addendum to FAR 52.212-5, includes FAR 52.232-19, Funds for the Next Fiscal Year will not be available (beyond September 30, 2004). The Government's obligation for performance of this contract beyond that date is contingent upon the availability of appropriated funds from which payment for contract purposes can be made.
