- This is a combined synopsis/solicitation prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offeror Representations and Certifications --Commercial Items; 52.212-4 Contract Terms and Conditions - Commercial Items; and 52.212-5 Contract terms and Conditions to Implement Statutes or Executive Orders - Commercial Items. NAICS Code is 423450. The proposed procurement is unrestricted. The U.S. Naval Hospital, Okinawa, Japan has a standing order requirement for 240 Cases (24 Units/Case) of Blood Collection Bags, 450ml (“Brand Name or Equal” with Terumo Medical Corp, CPDA-1 Collection Bag with Blood Sampling Arm). This blood bag features, subimposed label for easy identification of anticoagulant/collection volume; color-coded Blister Pack storage containers for easy identification of bag type; snap-to-open seal for ease in component preparation, easy-open peel tab outlet ports; needle cap with a tamper evident seal and bevel indicator; ultra-thinwall needle for added donor comfort; kink-resistant tubing, latex Free. Unit description: Quadruple Blood Bag, with 16 gauge ultra-thinwall donor needle, 450ml primary container with one 500ml Plus 5-day platelet bag and two 400ml satellite bags. Delivery shall be 12 cases per month to Tracy, CA. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Technical capability of the item offered to meet the Government requirement (including submission of descriptive literature); (2) Price; all to be submitted with proposal. Solicitation documents have been uploaded with offers due July 28, 2004. Contract effective date estimated to be October 1, 2004 for a period of one year. This solicitation is subject to availability of funds.
