Description

The Aberdeen Area Indian Health Service is currently seeking an Optometrist for a firm-fixed price,Small Business set-aside, non personal service contract to provide optomerty services at the Pine Ridge Service Unit to include the sites of Kyle Health Center, and Wanblee Health Center.The period of performance will from date of award to 12/31/04 or 175 days whichever comes first. The contractor will provide services to the Service Unit one (1) to four (4) eight hour days per week. The last date to request a copy of this RFQ package will be 7/21/04. Request for the RFQ-04-079 shall be made in writing or tele-fax at (605)-226-7669 and will be made available approximately 10 days after the date of publication of this announcement. NOTE: THIS NOTICE WAS NOT POSTED TO WWW.FEDBIZOPPS.GOV ON THE DATE INDICATED IN THE NOTICE ITSELF (14-JUL-2004); HOWEVER, IT DID APPEAR IN THE FEDBIZOPPS FTP FEED ON THIS DATE.
