Description

The VA Medical Center, Poplar Bluff, MO has a requirement for ambulance services. Contractor shall be required to furnish all personnel and equipment to provide emergent and non-emergent, twenty-four (24) hours per day, 365 days per year, ambulance services to beneficiaries of the John J. Pershing VA Medical Center, Poplar Bluff, MO. Services inlcude Basic Life Support and Advanced Life Support Ambulance Services for STAT, routine and scheduled trips. Contract will be for a Base Period of one year with option to extned 4 additional one-year periods. Anticipated effective date of services to begin is September 1, 2004. All responsible sources may submit a quotation which shall be considered by the agency. Request for quotation will be available on or about June 24, 2004. Due date for receipt of offers is July 26, 2004, 4:00 p.m. CST. This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Offerors are to refer to Addendum to FAR 52-212-1 - Instructions to Offerors for additional documentation to be submitted and a completed copy of the provision at 52.212-3, Offeror Representations and Certifications_Commercial Items, with its offer.
