Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, streamlined procedures for evaluation and solicitation for commercial items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. Solicitation number F7SVMP41100300 is issued as a request for quotation (RFQ). All responsible sources may submit a quote. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-19. The associated North American Industry Classification System (NAICS) for this procurement is 713940 and the small business size standard is $6.0 Million. This procurement is advertised as a 100 percent small business set-aside. The anticipated period of performance is 01 October 2004 through 30 September 2005. This procurement is subject to the availability of funds. Services Non-personal: Provide all labor, tools, parts, materials, facilities, and transportation necessary for aerobic instruction at the Columbus AFB Fitness Center in accordance with the Statement of Work. STATEMENT OF WORK GROUP EXERCISE: 1. Description of work. a. Scope: the Group Exercise Contract Manger (GECM) will provide the expertise needed to operate a well-rounded exercise program (i.e., step, floor, etc.) for the Columbus AFB Fitness Center as defined in this statement of work. b. Location: Classes are to be performed at Columbus AFB Fitness Center, Building 704, 829 Simler Boulevard. c. We have the requirement for a minimum of 15 classes per week to include but not limited to Aerobics, Spin, Strength and Tone, Pilates, Circuit training, Karate. Each class will be 1 hour to include warm-up, instruction and cool down period. 2. Personnel a. GECM will recruit and schedule qualified instructors who possess, as a minimum 6 months experience instructing large groups of 20 or more students and have one of the following national certifications, American Council on Exercise (ACE), American College of Sports Medicine (ACSM), Aerobics and Fitness Association of America (AFAA), or the Cooper?s Institute. NOTE: This last sentence is for AFI purposes only. If/when other organizations are recognized, they can be listed in the contract by an amendment. In addition, the GECM will have liability insurance and will ensure all instructors possess a current BLS or CPR card. All certifications must be valid prior to instructor beginning classes and must remain in effect throughout their period of employment with the contractor. b. The GECM will be responsible for the performance of work. An alternate, if available, shall be designated in writing to the Fitness Center Director (FCD) or Designee through the contracting office. c. The GECM and all instructors shall present a neat appearance and be easily recognized. Proper athletic attire will be worn at all times. Attire will be in good taste and subject to the approval of the Fitness Director. d. The GECM is reminded that off-duty, active duty military personnel hired may be the subject to permanent change of station, change of duty hours, or deployment. Absence does not constitute a valid excuse for nonperformance under this agreement. 3. Quality Control a. The contractor shall ensure all work performed under this Statement of Work (SOW) meets the criteria established in paragraph 7. 4. Hours of Operation. a. The GECM shall ensure certified group exercise instructors are available to conduct scheduled classes as specified. Note: Class schedule is subject to change as needed based on patron/program requirements. b. The GECM is required to provide instruction on federal holidays unless directed otherwise by the FCD. 5. Conservation of Utilities. a. The GECM shall instruct their employees in the utilities conservation practices and shall be responsible for operating under conditions, which preclude the waste of government utilities and resources. 6. Government Furnished Utilities and Services. a. The government shall provide, without cost to the contractor, the utilities necessary to accomplish the task(s) specified in this Statement of Work, fire protection, and 911 services. b. The government shall provide a designated area at each fitness center suitable for group exercise instruction. In addition, the government shall provide materials and equipment necessary to conduct classes (steps, dumbbells, mats), when government funds are available, with the exception of headset/microphones. Any needs which are not met and for which the government funds are not available are the responsibility of the contractor and their employees. 7. Specific Tasks. a. The contractor shall provide input on a master program schedule, on a monthly basis depicting group exercise instructor?s name and type of class. This master program must be submitted to the FCD or Designee at least 10 days prior to the effected month. b. The GECM must maintain a file on each instructor, which includes a copy of the individual?s certification, CPR/BLS, and first aid card. These files must be made available to the FCD or Designee and/or Contracting Officer upon request. c. The GECM must provide instruction for special events, such as an Aerobic Step-a?thon, Holiday aerobic marathon, and aerobic seminar during any given month. The FCD shall coordinate with the ACM at least one month in advance to express any special needs for the coming month. d. The GECM shall ensure each instructor incorporates standardized safe warm-up, stretching, and exercises techniques. A minimum of two (2) target rate checks will be performed during class, as well as, a safe cool down period to conclude the cardio segment of each class. Instructors will provide specialized instruction for students of each class. Instructors will provide specialized instruction for students to demonstrate high and low techniques/options. e. The GECM will conduct a quarterly survey of class participants to collect feedback on quality of instructors and instruction given during the preceding quarter. Survey materials shall be provided to the GECM. Results of survey shall be summarized and submitted to the Fitness Director upon demand. f. The GECM shall conduct monthly communication with all instructors to keep them informed of facility/base policy changes or any other information needed to ensure a quality program. This will be at no cost to the government. g. The GECM shall provide and ensure instructors maintain an instructor sign-in log. Instructors must sign-in prior to the start of each class. h. The GECM shall provide and ensure instructors maintain a daily attendance log, which includes his/her name, date and class time. As well as a breakdown of active duty, dependents, Weight and Body Fat Management Program, Fitness Improvement Program; self-paced and monitored personnel attending classes. A sample roster will be provided by the FCD. Rosters will be kept in a three ring binder and maintained on the sign-in desk in the group exercise room. This log must be available to the FCD or Designee upon request. i. The GECM will consolidate daily attendance rosters into a one-month report utilizing the same categories outlined in Paragraph 7 h, and shall provide this information to the FCD at the end of each month. j. The GECM shall ensure instructors arrive at least 10 minutes prior to their scheduled class time to prepare to instruct. k. The FCD or designated representative shall verify completion of work under this agreement. l. The GECM shall ensure instructors are aware that they must notify the on-duty fitness center staff of any injuries or accidents associated with the group exercise customer. Should ambulatory services be needed, the fitness center staff shall notify the proper entity for assistance. If the fitness center staff is not readily available, the group exercise instructor has the authority to call 911. m. The GECM will ensure instructors use a sound system and microphone and be familiar with their operation. n. The GECM will ensure an instructor will be provided for all scheduled classes or a qualified substitute as outlined in Paragraph 2 a., if the primary instructor cannot teach. Except for a bonafide emergency, the FCD or Designee must be notified one day prior of a substitute instructor. The classes include but are not limited to aerobics, spin, strength, tone, pilates, circuit training, and karate. Total number of line items: 2. LINE ITEM: 0001 ? BASIC AEROBIC INSTRUCTION ? Interested parties capable of providing the above must submit a written quote to include company name, address, telephone and fax numbers, email address, business size/type, unit price and estimated total cost, Commercial and Government Entity (CAGE) code, DUNS number, tax identification number, and discount terms. All interested parties must be registered with the DoD Central Contractor Registration (CCR) database as prescribed in FAR 52.204-7 and DFARS 252.204-7004 (Alt A). The following clauses and provisions can be viewed through Internet access at the Federal Acquisition Regulation website http://farsite.hill.af.mil. FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. Offerors shall complete and present with their offers FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (July 2002). In addition, the clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this solicitation. Specifically, the following clauses cited are applicable: FAR 52.222-26, Equal Opportunity; FAR 52.232.33, Payment by Electronic Funds Transfer- Central Contractor Registration. Award will be based on meeting the requirement, evaluation of lowest price, and responsible offer/quote. The due date for all quotes is 22 July 2004, 3:30 p.m. Central Standard Time (CST).
