Description

Provide all personnel, equipment, and supplies for ambulance transportation of patients at the Department of Veterans Affairs Medical Center, Detroit, MI. Projected period of service is base year through 8/31/05 with the possibility of four pre-priced option years. Request for proposals (RFPs) 553-53-04, shall be submitted in writing via mail or facsimile at (313) 576-1077. All requests shall include company name, address, contact person and facsimile number. Awarded contractor shall be prepared to begin providing service at time of award. No electronic requests will be accepted as VA does not assume any risk associated with use of electronic mail. Proposals are due by 4:00 pm local time, Friday, July 30, 2004. Proposals received after the due date will not be considered. Contractors are responsible to verify receipt of any proposals sent to the contract office. Always refer to RFP 553-53-04 on all documentation and requests.
