- Description
- The Federal Aviation Administration, Mike Monroney Aeronautical Center (MMAC), 6500 S. MacArthur Blvd., Oklahoma City, OK, intends to establish a Qualified Vendors List (QVL) to be used for fabricating corrugated plastic shipping containers. To be considered for inclusion on the QVL, the following minimum qualification requirements of this initial Screening Information Request (SIR) must be met. Provided as an example are photographs of a corrugated plastic shipping container (Attachments 2-6). Corrugated plastic shipping containers will be procured on an as-needed basis. Dimensions will vary and will be specified in the purchase description when the requirement for a container arises. (A) The QVL is a 100 percent set-aside for small business concerns. The FAA Acquisition Management System defines a small business as, ??????a business, including it???s affiliates, that is independently owned and operated and not dominant in producing or performing the supplies or services being purchased, and one that qualifies as a small business under the Federal Government???s criteria and North American Industry Classification System (NAICS) size standards.??? For this procurement, vendors must be classified as a small business under NAICS code 322211 (500 employees). Vendors shall complete and submit the Business Declaration Form (Attachment 1) with required documentation. (B) Vendors shall submit a description of their capability to meet the following four criteria: (1) Manufacturing Capabilities ??? fabrication and manufacturing of corrugated plastic shipping containers; (2) Past Performance on Government Contracts ??? provide contract number(s) and name and phone number of point(s) of contact. The FAA reserves the right to contact references and verify each vendor???s information; (3) Computer Aid Design (CAD) Capabilities ??? capability in-house or have sources for software and firmware needs; and (4) Inventory - sufficient space for storage of drawings (physical and computer). To receive consideration, all documentation required in the previous paragraphs must be provided, along with a name and telephone number of a point of contact, to the following address: FAA/MMAC, P.O. Box 25082, Oklahoma City, OK 73125, Attn: Joy Dickson, AMQ-210, with reference made to Qualified Vendors List File #64-1, by July 28, 2004. If required, a subsequent SIR will provide further qualification criteria to enable the FAA to evaluate and then down-select vendors for the QVL. The FAA reserves the right to limit the number of vendors selected for the QVL. Data submitted that is found to be false will result in permanent removal from consideration for this QVL. The FAA reserves the right to conduct an on-site survey with companies interested in this requirement. Special consideration may be given to woman-owned small businesses and service-disabled veteran small businesses for participation in the QVL. Re-establishment of this QVL will be a unilateral decision of the FAA and will be reviewed annually. The FAA reserves the right to include additional vendors on the QVL. The decision to remove a vendor from this QVL will be based on performance and the ability to be competitive.
