Description: This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. The announcement constitutes the only solicitation; proposal s are being requested and a written solicitation will not be issued. DAHA25-04-Q-0017 is the reference number for this Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acq uisition Circular. The solicitation has been set-aside 100% for small business. The NAICS Code is 811219. Services, Non-Personal. To provide all equipment, tools, parts, material, labor and transportation necessary to inspect, perform functional checks and repair Manual Reverse Osmosis Devices (MROD). The contractor will be responsible for performing operational inspection of the units In Accordance With (IAW) Air Force Technical Orders (T.O.s) 40W4-16-1C (OPN AND MAINT W/IPB -- MANUAL REVERSE OSMOSI S DESALINATOR, ROD-06-LL-1, NSN 4610-01-313-6085 (RECOVERY ENGNG) (MROD-06-LL-1) and 40W4-15-1C OPN AND MAINT INSTR W/IPB -- MANUAL REVERSE OSMOSIS DESALINATOR, MROD-35-LA-1, NSN 4610-01-313-6086 (RECOVERY ENGNG) and applicable manufacturers data. In addi tion, all units will be provided a salt water flush and pressure test, repair internal leaks and replace membrane as needed, fresh water flush and biocide dip, and install hoses and clean unit as needed. The 155 OSF/DOL will be responsible for providing c urrent T.O.s to the contractor and will be responsible for immediately informing them of any changes. Visual Inspection: Units will be unpackaged and the contractor will perform a complete visual inspection prior to the operational inspection. This inspec tion will consist of checking for physical damage to the unit, missing components, or weathering. Any unit with missing parts or evidence of damage will be identified to the user (155 ARW Life Support). CLIN 0001: 4610-01-313-6085 Small Reverse Osmosis Pump. Quantity of 50 each. CLIN 0002: 4610-01-313-6086 Large Reverse Osmosis Pump. All items to be delivered Fob Destination, Lincoln NE 68524. Delivery time no later than 90 days after receipt of or der. The following FARS are applicable to this order: 52:212-1, Instructions to Offerors-Commercial Items; 52.212-2, Evaluation-Commercial Items (price and or equal determination); 52-212-3, Offeror Representations and Certifications-Commercial Items; 52 .212-4, Contract Terms and Conditions-Commercial Items; 52.212-5, Contract Terms and Conditions Required TO Implement Statutes or Executive Orders-Commercial Items; 52.247-34, FOB-Destination; and 52-225, Buy-American Act. All quotes are due no later than 11:00 a.m. on Friday, July 16, 2004. The only FAR requirement to be returned with the quote at this time will be 52.212-3 (fully completed).
