Description
Provide all labor, equipment, material, tools and qualified supervision to Expand and Modernize the emergency department and to construct a new multipurpose facility (auditorium) on the second floor at the Jesse Brown VA Medical Center, 820 S. Damon Ave. Chicago, Illinois 60612. SCOPE OF WORK: The contractor shall furnish all materials, equipment and personnel necessary to construct a new two story addition of 5000 SF each floor, demolition of approx. 100 SF structure and, in addition, renovate approx. 11,800 square feet of existing space on the first floor, second floor and third floor of the main hospital. The project includes expansion of Emergency Room to newly built first floor and renovation of existing first floor areas to modernize and expand the emergency department by increasing the size of the waiting area, increasing the patient capacity from 8 to 14 beds, constructing a new seclusion room and construction a 2-3 bed chest pain evaluation unit. The existing and additional expansion space is to be reconfigured for the most efficient use of space to enhance patient privacy and confidentiality and to reduce ?door-to-discharge? time. The modernization may include new flooring, walls, ceilings and electrical and mechanical systems. Second floor construction of this project includes construction of a new multipurpose room (auditorium) with full height folding partitions to divide the large room into three medium size meeting rooms, storage rooms, and renovation of adjacent rooms on second and third floors of existing building. Construction of this project will be phased to continue current operations around the construction area in a healthcare environment in conformance to the contract documents and all applicable codes and standards for Department of Veterans Affairs, Jesse Brown VA Medical Center, 820 S. Damon Ave. Chicago Illinois 60612. This requirement is an 8(a) competitive set-aside, geographically restricted for firms serviced by the Small Business Administration (SBA), Illinois District Office. It is determined that competition will be limited to 8(a) firms serviced by the Illinois District Office. All other firms are deemed ineligible to submit offers. The SBA, Chicago District Office shall provide the VA a list of all qualified Illinois firms, for participation in this procurement process. All other interested parties should address their concerns to the SBA, Chicago District Office, 500 W. Madison Street, Suite 1250, Chicago, Illinois 60661-2511, attention Mr. Robert Conner, telephone (312) 353-4528. No telephone requests for the solicitation will be accepted. The completion date is 365 days from Notice to Proceed. Cost range is between $4,800,000 and $5,400,000. NAICSC code is 233320. Issue date is on or about July 12, 2004. Award is subject to the availability of funds.
