Description
Red Rock Job Corps Center, Sullivan County, Route 487N, Lopez, PA 18628 is soliciting proposals for the erection of a new glass lined, steel bolted wastewater tank, approximately 28'x13.14' with a capacity of 72,000 gallons. Contractor will dismantle tank in operation now and dispose of legally. Contractor shall provide a temporary sludge holding tank for the period starting with dismantle until complete erection of new tank and online for use. Contractor will be responsible for all permits, inspections and fees required by local, city, county or state. Contractor shall meet or exceed all codes for this project. A MANDATORY WALK THROUGH will be held on July 22, 2004 at 10:00AM at the Visitor's Center at Red Rock Job Corps Center. Construction Bid Packages will be distributed at the walk through only. BIDS will be due AUGUST 5, 2004 by 3:00PM with BID OPENING to follow immediately. Estimated cost: $25,000 to $100,000. NHJCC Daily - Sign off on each students Job Corps Training Achievement Record (TAR). - Supervision by a licensed Registered Nurse in the State of Connecticut. B. Course Description The purpose of the Certified Nurse's Assistant Program is to provide information and skills to enable the student to provide basic, safe, and quality care for patients in health care settings. Students will learn skills and theory required to pass the State Nurse Aide Exam and work as entry level providers. C. Course Objectives Students will master the following: - recognize the role of health care systems and the role of the nursing assistant within this system - encourage patients to maintain and regain independence - support and maintain patients rights - to be sensitive to the physical, emotional, spiritual, and psychological needs of those requiring health care - effectively communicate with staff, patients, and families - provide basic quality health care - chart patients conditions and changes - practice infection control and be aware of all safety regulations - take, measure, and record vital signs and understand normal values - Clinical is taught at an area Long Term Care facility. Instructor will notify student of location during class - CPR Training & Certification. D. Graduation A Certificate of Completion will be awarded for all Programs following successful completion of all modules of the individual course. A student must have a score of 70% or better in each academic module and exam to progress and complete the course. In addition, all financial obligations must be met. E. Student Services Students will qualify for placement assistance upon graduation to include sample resume, interviewing techniques, and an "Open House" for graduation and information regarding potential employers will be provided. Timeline: June 20th, 2004 - Bid Packages Available for Pick Up June 25th, 2004 - Bidder's Conference @ 10:00am July 25th, 2004 - Due date for proposals August 15th, 2004 - Internal Review of Proposals August 15th, 2004 - Request for Approval from Corporate Office Department of Labor. August 31st, 2004 - Contractor Notification of Selection September 1st, 2004 - Contract Start Date A complete copy of the Request for Proposal (RFP) including additional requirements, objectives, and responsibilities as wekll as other documentation including Job Corps manuals and Technical Supplements may be reviewed in the office of Mr. William Johnson, Purchasing Agent.
