Description

The VA Black Hills Health Care System with facilities located in Hot Springs and Fort Meade, SD intends to award a contract for a Community Based Outpatient Clinic (CBOC) to provide Primary Care Services on an outpatient basis to veterans living in and around Todd County,in South Dakota. The CBOC must be located in Todd County and preferrably in Mission, SD. It is anticipated that during the base contract period there will be approximately 805 visits per year with an increase to approximately 1074 visits at the end of the five year period. Reimbursement shall be per episode of care. This contract shall be for a base year with four additional option years. Contract base year period shall be from November 1, 2004 through October 31, 2005. Contractor's clinic must be ready to serve VA veterans on November 1, 2004. The CBOC shall be responsible for managing the veterans' primary medical care. Referrals for sub-specialty care and hospitalization shall be made to the VA Black Hills Health Care System. Receipt of offers is due by close of business July 29, 2004 at 4:30pm. A Pre-Bid telephone conference will be scheduled at a later date.
