DESCRIPTION: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation document will not be issued. The Veterans Affairs Medical Center Sheridan, Wyoming intends to enter into a firm fixed price contract wherein the Contractor is to furnish all components and services necessary to upgrade the existing Dukane Pro Care 4000 Nurse Call System to the Dukane Procare 6000 Nurse call System at the V A Medical Center, Sheridan, WY. Work to include replacing the existing head end equipment, remove and replace existing patient stations with new three gang dual patient stations with two auxiliary inputs, remove and replace the existing master stations with new master stations and touch screen controls, complete connections, program test, and certify the system. Installation under this agreement also to include additions on new stations in locations specified on the drawings (contact the Contracting Officer for a set of Drawings), Request for Quotations (RFQ) # 666Q-04-06 entitled "Nurse Call System" is hereby issued for commercial items as an Unrestricted solicitation and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, FAR Part 13. This announcement incorporates provisions and clauses in effect through FAR FAC 2001-22 and VAAR 97-10. NAICS Code 517211 applies for this procurement and the small business size standard is 1,500 employees. Contractor's quotes are herein requested. A response to the requirements listed below shall be submitted on contractor's letterhead or formal quotation form to include pricing for all line items in the quotation schedule. Contractor's price quote must contain the name; title and signature of person authorized to submit quotes on behalf of the contractor, and agree to be bound by all the requirements under this RFQ. The contractor will provide the following services in accordance with requirements of the contract: 1. DELIVERIES OR PERFORMANCE The contractor has forty five (45) calendar days after date of award to complete the installation. 2. PAYMENTS Payment will be made after the completion of all work and upon receipt of properly prepared invoice for the completed work. Invoices are to be submitted to the Contracting Officer, VA Medical Center (90C), 1898 Fort Road, VA Medical Center, Sheridan, WY 82801. Contractor agrees to accept payment via Electronic Funds Transfer between the US Treasury and the Contractor?s designated bank account. Contractor agrees to complete SF 3881,and return it within five days of notice of contract award. 3. CONTRACT ADMINISTRATION DATA The Contracting Officer as the agent of the Government, with authority to negotiate terms, execute legally binding agreements affecting price, quantity, or quality of performance under this contract and administer contracts on behalf of the Government. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in costs incurred as a result thereof. 4. CONTRACTING OFFICER?S TECHNICAL REPRESENTATIVE He/she is the technical advisor to the Contracting Officer and is responsible for the daily technical oversight of the Contractor?s work and for making recommendations to the Contracting Officer regarding matters of contract administration. The foregoing is not to be construed as authorization to interpret or furnish advice and information to the Contractor relative to the financial or legal aspects of the contract. Enforcement of these segments is vested in and is the responsibility of the Contracting Officer. 5. CURRENT CONTRACT Contractor must state whether they currently hold a contract with another VA Medical Center, Federal or State Installation. YES__ NO __. If yes indicate the address of the installation ________________________________________________________________. 6. WARRANTY Contractor to provide a one year full service agreement (100% Labor and Material) as part of this agreement. 7. IN-SERVICE Contractor to provide in-service and training for maintenance and operation of installed equipment under this agreement. 8. DRAWINGS Contractor to provide As-Built Drawings in CAD under this agreement.
