- This Amendment covers the above referenced solicitation and is hereby amended as follows: A. Deadline for Questions and answers is Thursday June 10, 2004, at 12:00 Noon Chicago time. B. The proposal due date has been extended to June 17, 2004 at 4:00 p.m., Chicago time. C. The Unit Price Sheet must be submitted with the Standard Form 1442. D. Bricker Federal Building (Chicago) Team Members: Mr. Panos Giannakopoulos, GSA Project Manager, Mr. Mike Russell, URS Project Manager, Ms. Bobbie Tyler, Contracting Officer E. You must acknowledge receipt of this Addendum No. 1 as indicated in block 11 of the Standard Form 30 prior to the date and time for receipt of proposals. ADDENDUM NO. 1, June 8, 2004 TO ALL DOCUMENT HOLDERS: John W. Bricker Federal Building Addendum No. 1 covering the subject project and addressing all questions/clarification requests received through Monday June 7, 2004. Please retain this with your Documents. ADDENDUM TO THE CONTRACT DOCUMENTS FOR THE JOHN W. BRICKER FEDERAL BUILDING 1.1 GENERAL A. This Addendum covers changes to the Documents will become a part thereof. B. These items modify only the portion of the Documents specifically noted. All other wording and Drawings remain in effect. C. Each Contractor shall include these items to the extent they affect his quote. D. Acknowledgement of Amendment No. 1 issued March 9, 2004, need not be acknowledged. E. Acknowledgement of modifications numbered 1 thru 6 dating from the period of March 11th 2004 through May 13th 2004 on the bid form need not be acknowledged. F. The Unit Price Sheet was inadvertently left out of the bid documents. See attached Unit Price Sheet for use in submitting your bid for garage repairs G. No clarifications will be accepted with the bid form. H. All bids shall be submitted at the time and place stated in the documents I. There is no Page 3 of 3 on Attachment A to Standard Form 1442. This page will be for GSA use only. J. The applicable ?Minimum Hourly Wage Rates? are included on the original CD of documents. K. For Builder's Risk Insurance requirements, reference the Construction Contract Clauses FAR 52.228-2, 52.228-5, and page 2 of Section 00800 Supplementary Conditions Part 3 - Insurance. L. Attachment A to Section 00120, Item 4 & 5, the contractors shall be evaluated based on their experience on occupied office buildings. However the GSA may consider experience in hospital type buildings if their office renovation experience is not adequate. 1.2 PROJECT MANUAL A. Section 01100, Summary 1. The Scheduling and Phasing plans included in the documents are samples. It is up to the contractor to revise them to suit their best planning strategy to accomplish the project. It is acceptable to work in more than one tenant quadrant at a time to perform the renovation work as long as the tenants in any particularly quadrant are not disrupted for more than 6 weeks at a time. 2. The anticipated date for Notice of Award and Notice to Proceed are as follows a. August 2004 - Notice of Award b. July 16, 2004 - Notice to Proceed 3. Paragraph 1.3, the listed projects entail miscellaneous office layout renovations including addition and/or removal of walls and the associated revised flooring and ceilings. B. Section 01260, Options 1. Paragraph 3.1,P, The additional requirements of the specification sections referenced in this option are only to be included in the Option 16 bid price. C. Section 01310, Project management and Coordination 1. Part 2, Add the following: the contractor shall be responsible to provide the electronic communications software tool for the team. The team shall include access/license to the system for six users outside of the needs of the contractors personnel. The product used shall be Constructware, and it shall be capable of providing an integrated centralized technology solution for managing the business processes and work flows of the construction lifecycle and improve the construction lifecycle through technology innovation during the pre-construction through project closeout. A two hour training session for the GSA and their CM contractor shall be provided. D. Section 01311, Critical Path Method Scheduling 1. All scheduling shall be completed electronically on Primavera, Sure Trak, version 3.0a E. Section 15050, Basic Mechanical Material and Methods 1. Paragraph 1.6 ?G.2, It is not necessary to purchase mylar backgrounds, the provided electronic CAD files, shall be used. 1.3 PROJECT DRAWINGS A. The notes on the floor plan drawings indicate where new finishes are required. No finish schedule will be issued B. Drawing No. S 1 01 G thru S 108 G A2-1, 1. Plan note 4 on structural drawings, regarding crack repair, states "See repair types 1 and 2 on sheet S 0 02 for required repair procedures". Repair type 1 is for cracks wider than 0.08" and repair type 2 is for cracks narrower than 0.08". Please advise which cracks are wider than 0.08" and which cracks are narrower than 0.08" C. Drawing No. A-7-B1 through A-7-07, 1. Note #16 - disregard Note #16 on all plan sheets. The referenced louver was eliminated. D. Drawing No. A-1-01, Floor Plan 1. The chair rail referenced for room #114, shall match existing, and consist of less than eight lineal feet of hard wood chair rail with stain and varnish finish. E. It appears that there is a conflict between the door schedules and the plans. Final payment to the contractor will be based upon actual approved work in place. Additions to or deductions from the lump sum shall be at unit prices listed on the bid form. B. Unit prices shall be applicable to an increase or decrease in the scope of work by 25%. Unit prices will be established by negotiated agreement for quantity variations in excess of 25%. (Note, there?s nothing magical about 25%, I would recommend a minimum of 15%). C. The government will retain the services of an independent testing agency to measure actual work performed by the contractor. The testing agency?s measurements will be the basis for determining actual quantity of work performed by the contractor. D. Quantities provided on the bid form were calculated from the contract drawings for the contractor?s convenience. The contractor should refer to the contract drawings for additional information regarding the repairs, including location, size, shape, depth, and repair procedure. E. The contractor shall be given credit for a minimum of one square foot of floor patch at each individual floor patch location regardless of the actual size of the floor patch. ALL DOCUMENT HOLDERS WILL RECEIVE A HARDCOPY OF THE STANDARD FORM 30 BY JUNE 10, 2004. END OF ADDENDUM NO. 1
