- This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR Subpart 12.6, and supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Qu otes are requested; however, a written solicitation will not be issued. Solicitation number W911S8-04-T-0040 is applicable and is issued as a request for quotations. The solicitation and incorporated provisions and clauses are those in effect through Fed eral Acquisition Circular 2001-15. The requirement is small business set-aside with a NAICS Code of 332312, and size standard of 500. The description of the commercial items is as follows: Clin 0001: Fabricated Pivot Support Arm; Quantity: 250 each. Plea se e-mail to request sample drawings at: Barbara.Kinnear@lewis.army.mil. State if FOB is Destination or Origin. If Origin, state shipping charges. Delivery is to be as soon as possible to DOL, Bldg 9580, Fort Lewis, WA 98433-9500. Please state Estimated Delivery Date. Partial Shipments will be acceptable. Any award resulting from this request for quotations will be made to a responsive, responsible quoter using a SF1449 form. Quotes will be evaluated on a lowest price technically acceptable competitiv e acquisition evaluation factor whose offer conforms to the requirements as shown in the sample drawings, and who has acceptable past performance. Once the lowest price technically acceptable quote has been determined, a sample of the item shall be provid ed to the Government within seven (7) working days for approval prior to additional items being fabricated. Award will be made on an all or none basis. If quotes are provided for an equal item, specifications must be submitted. Provision 52.212-1, Instru ctions to Offerors-Commercial Items, applies to this acquisition. An addendum to this provision is applicable, in which the term offeror (whenever it appears) shall be replaced with the term quoter. Quoters are required to submit completed copies of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their quotes. FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and does not include any addenda. Far clause 52.2 12-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, applies to this acquisition. The following clauses within 52.212-5(b) are applicable to this acquisition: 52.222-3, 52.222-19, 52.222.21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-33. The following clause within DFARS clause 252.212-7001(b) is applicable to this acquisition: 252.225-7001.
