Description

Darnall Army Community Hospital, Fort Hood, TX has a requirement for the purchase of Lumbar Translateral Interbody Fusion (TLIF) Bone and Cervical Ring Allograft commonly used in hospitals performing spine surgery. The estimated quantity to be deli vered per year is 300 Lumbar TLIF Bone & 50 Cervical Ring Allograft. All products are to be placed in the hospital on a consignment basis. The period of performance is from the date of award through 30 September 2004 with two option years. This requirem ent will be on an as needed basis. This procurement is being conducted as a commercial item acquisition in accordance with FAR part 12. This requirement is unrestricted, all responsible sources may submit an offer, which will be considered. Solicitation number W81K00-04-T-0359 will be issued on or about 21 June 2004 and will be available at https://acquisition.army.mil/asfi/gov_personnel_main.cfm. Paper copies of this solicitation will not be issued and telephone requests or fax request will not be accepted.
