Description

The GLAC (Great Lakes Acquisition Center) is soliciting quotations to provide Positron Emission Tomography, Computerized Tomography (PET/CT) Mobile Trailer to provide PET/CT scans, and services of a PET/CT Technologist (plus necessary support staff as patient volume dictates) for the Veterans Administration Medical Center, Milwaukee, WI. (VAMC), 5000 W. National Ave., Milwaukee, WI 53295 in accordance with the specifications listed herein. The contractor will pay for AGFA and PET/CT Scanner OEM expenses to interface the PET/CT scanner with the VA AGFA PACS system. Also, the Contractor shall provide all preventative maintenance/repairs for the PET/CT mobile unit. All work performed under this contract will be completed at VA. The scanner will be located at VAMC, Milwaukee, WI. This contract period shall be for the period of July 01, 2004 through June 30, 2005 with the option to renew for two (2) additional (1) year periods, at the Government's discretion. Contract type is firm-fixed price. This is a total 100% un-restricted and offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications-Commercial Items. The NAICS code is 621512, size standard $11,500,00 applies. Electronic version and hard copy of solicitation will be available on or about May 24, 2004.
