REQUEST FOR INFORMATION FROM HEALTH ACTUARIAL FIRMS The Defense Human Resource Activity, Office of the Actuary, Arlington, VA is considering commissioning a study by a health actuarial firm to analyze and provide cost projections related to TRICARE, the Department of Defense health program for military beneficiaries. We are seeking information regarding the type of analysis and approach health actuarial firms could provide, and the feasibility of being able to deliver results within a 2-3 month timeframe. If it is infeasible to complete a comprehensive study and provide a final report within a 2-3 month timeframe, we would like to know what type of interim or draft results could be provided. We would also like to know the requirements in terms of the type of Military Health System (MHS) data that would need to be provided to the firm. TRICARE benefits are provided at military hospitals, clinics and pharmacies (the direct-care system), and purchased from the private-sector care through the Department?s insurance component (purchased care). The ultimate desired output is a 5 to 10 year projection of Defense Health Program (DHP) costs to care for over 8 million eligible DoD beneficiaries to reflect the firm?s best projection of expected utilization and cost increases for current users of the system, utilization and cost increases related to the increase in the number of users, advances in medical practice and medical inflation. Of particular interest is the recent phenomenon referred to as the ?return of previously non-reliant? (or ?ghost?) beneficiaries to the DoD Military Health-Care System. The growth in ghost beneficiaries are expected to lead to significant cost increases. The ghost effect largely relates to the growing relative attractiveness of the TRICARE benefit compared to alternative plans. The types of useful analysis and results that are desired include: Analysis of the TRICARE benefit as compared to private sector plans. This could be done both qualitatively and quantitatively, e.g., assigning a relative value measure to the TRICARE benefit compared to a typical private sector plan. Assess the actuarial value of the TRICARE benefit compared to civilian plans. Analysis of historical cost trends, reasons for the cost increase, and projected cost of the DHP due to the value of the TRICARE benefit analysis from above. For example, TRICARE offers a comprehensive benefit with low beneficiary cost-sharing amounts that have not been indexed over time. Analysis potentially would include assessing all MHS beneficiaries? propensity and need for enrollment in and use of other (non-DoD) private insurance and expected ultimate percentage of eligible beneficiaries who would use TRICARE and to what extent. Analysis of the effect of the recently-enacted TRICARE for Life (TFL) benefit for Medicare-eligible DoD beneficiaries on costs for non-Medicare-eligible beneficiaries. For example, the existence of the TFL benefit eliminates the requirement certain under-65 retirees may previously have had to maintain other coverage in order to have continuing insurance coverage upon attaining Medicare eligibility. The potential effect is to increase TRICARE costs for the non-Medicare-eligible population. Analysis of the worst-case scenario, i.e., costs if all eligible MHS beneficiaries dropped all other coverages and relied exclusively on TRICARE. Analysis of other factors leading to cost increases, such as would be seen in other market plans. We are not committing to conducting this study; we are only seeking information and interest at this time.
