Description
The Health Resources and Services Administration (HRSA) is conducting a Sources Sought/Market Survey to locate Small Businesses, HUBZone, and/or 8a firms under NAICS Code 541611, capable of performing Analytic Support.***The purpose of this contract is to use quality improvement measures to improve care for children and youth with special healthcare needs through the implementation and dissemination of the Medical Home concept using the Learning Collaborative methodology. This contract will be conducted through two concurrent and interwoven components: 1) strengthening State Title V capacity to support implementation and spread of the Medical Home, and 2) leading a Medical Home collaborative for primary care practices.***The primary responsibility of the recipient of this contract will be to provide knowledge, skills and expertise in the use of learning collaborative methodology to conduct two concurrent components. Component 1 to include: identification of state Title V teams, pre-work with state teams around quality improvement methodology, tools for assisting primary care teams in the implementation of Medical Home, the development of strategies for statewide spread, and principles of family ?professional partnership. Component 2 should include: three Learning Collaboratives with at least two of them being in-person sessions, provide training materials, coaching and faculty leadership throughout the Learning Collaborative process to both Title V teams and the primary care practices, and the collection and evaluation of formative data to measure success of the quality improvement process.***The organization must have extensive experience in quality improvement methodologies utilizing learning collaboratives as a strategy with specific emphasis on the concept of Medical Home. The organization must possess extensive experience working with state Title V agencies and primary care practices around quality improvement measures. The offeror must possess in-depth knowledge of the Medical Home concept and a proven ability to implement these standards at the state Title V and primary care level. The offeror must also demonstrate a proven ability to work with families and professionals around partnership in care of children and youth with special health care needs in the Medical Home and community. The contractor must demonstrate its ability to collect and evaluate the outcomes of the improvement process over time.*** Interested organizations must demonstrate and document, in any Capability Statements submitted, extensive experience with, and the ability to perform, all the tasks mentioned above. Organizations should demonstrate capability to administer and coordinate interrelated tasks in an effective and timely manner. Documentation may include, but not be limited to, both Government and commercial contracts, providing references (with names, telephone numbers, and email address) and any other information serving to document the organization's capability. The Government anticipates awarding a two-year contract. This is not a Request for Proposals and does not commit the Government to award a contract now or in the future.
