Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations document (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W91253-04-R-6015 is issued as a Request for Proposal (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-15. This procurement is for the purchase of Political Advisor services, on Eagle Base-Tuzla, Multinational Brigade-North, Bosnia-Herzegovina. Period of Performance shall be 5 months. All work shall be completed in accordance with the Performance Work Statement (PWS) at Attachment 1. QTY 5 months CLIN 0001, Political Advisor services, with a period of performance to begin on 1 August 2002 ending 31 December 2004. Specifications: See Attachment #1, PWS, dated May 13, 2004 Dates: Period of Performance: 5 months beginning 1 August 2004 ? 31 December 2004. Options may be used if required. It is the intent of the government to award the contract on or about 28 May 2004 in the interest of continuity and to allow adequate preparation time for successful offeror transition. The contractor is expected to commence work on 1 August 2004. Delivery: All prices are to be quoted F.O.B. destination: Joint Contracting Center Eagle Base-Tuzla APO AE 09789 FAR 52.212-1, is hereby incorporated by reference. ADDENDUM TO FAR 52.212-1, the following paragraphs are amended as follows: (b) is amended to add: Submission of offers by Fax or Mail IS NOT authorized. Send signed, completed Blocks 17a, 26, 30a, 30b, and 30c of SF 1449, and scanned offers via E-Mail only to Frank.Graham@email-tc3.5sigcmd.army.mil. and (b)(6) is amended to add: Offerors are to fill out prices for CLIN 0001 provided as Attachment #3 with submission of their offers. FAR 52.212-2 is hereby incorporated by reference. ADDENDUM TO FAR 52.212-2 (a) is hereby amended as follows: Evaluation Criteria: The Government will award a Firm Fixed Price contract resulting from this RFP to the offeror who, conforms to this offer and, based upon Past Performance Tradeoff (PPT), provides the government with the best value. Factors are: 1) Past Performance (signified by former employer-completed questionnaires) and 2) Price. *Past Performance shall be significantly more important than price. The failure of an offeror to provide information sufficient to determine conformance with the solicitation may result in the offeror being ineligible for award. Questionnaires should only be completed for ?relevant services performed since 14 May 2001 or later?. Offerors are asked to submit at least 2 questionnaires. Offerors who fail to provide past performance will be considered non-responsive and removed from the evaluation. Offerors must submit written proof that clearly shows they have met the following MANDATORY REQUIREMENTS: (Any mandatory requirement not being met results in offeror being excluded). 1) Have at least 3 years of experience working in a Political Advisor-related position 2) Possess strong communication skills 3) Have a current DoD-recognized TOP SECRET clearance Offers shall include a completed copy of the provision at FAR 52.212-3, 'Offeror Representations and Certifications-Commercial Items', (found at http://www.arnet.gov/far/farqueryframe.html) with their offer. The clause at FAR 52.212-4, 'Contract Terms and Conditions-Commercial Items', applies to this acquisition and is hereby incorporated by reference. ADDENDUM TO FAR 52.212-4. The specific paragraphs of FAR 52.212-4 that have been tailored are: Para (b) Assignment. The following clause is hereby incorporated by reference: DFARS 252.232.7008 - ASSIGNMENT OF CLAIMS (OVERSEAS) (JUN 1997) (Reference 32.806(a)(1)) ELECTRONIC FUND TRANSFER: Payment for goods or services received may be made by check or by Electronic Fund transfer. In order to ensure prompt and accurate payment, your invoice must contain your bank name, address; account number and bank routing number and check remittance address. ENGLISH LANGUAGE DOCUMENTATION: All contractor-prepared correspondence furnished under this contract shall be written in the English language. Para (p) Limitation of Liability. The following clause is hereby added: LIABILITY. For the purpose of this contract, the Contractor is considered as an independent contractor and not as an agent of the U.S. Government. During performance, the Contractor shall be liable and will indemnify and hold harmless the Government, its agents and employees against all actions or claims for all damages to persons or property, including death arising or resulting from fault, negligence, wrongful act, or wrongful omission of the Contractor, his agents or employees. 52.252-2 CLAUSES INCORPORATED BY REFERENCE (FEB 1998) This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text.
