- Architect and Engineering Services ? Construction ? Potential Sources Sought: A/E to provide design service review: to review any and all design documents, drawing, specifications, products and materials submitted by antenna A/E. This work will be performed at Northport VA Medical Center, Northport, NY. Service Review includes the following: review specified products installed to assure compliance to materials used in projects; review lead abatement specifications, including the use of any environmental barrier protection devices, grinding tools and vacuums; specify lead removal procedures for limited lead removed and procedures for lead removal exceeding minimum amounts removed; review lead removal procedure for conformance and compliance to OSHA, VA and NYS removal regulations and requirements; review lead disposal waste manifests and verify lead was accepted by the landfill; review specified personnel protective devices, including personal protective equipment for lead removal workers and fall protection devices; review specified proposed lead environmental testing procedures, acceptance and action levels; review qualifications, training records of employees removing lead; review qualifications industrial hygienist utilized for environmental lead testing; review all welds and weld sizes for structural stability; review welding procedures, minimum qualifications of welders, specify minimum welding qualifications; review planned attachment of any conduit, wire, cable to tower; review conduit specifications and sizes. Propose larger if anticipated growth is expected. Review all proposed wiring and wiring sizes; review proposed trenching, depth of trenching, conduit material and trench backfill; review construction design site safety features (temporary fencing, fall protection personnel protection devices etc.); review proposed location of antennas and equipment to eliminate safety/tripping/obstruction other hazards; review dielectric fittings (galvanic action) all ferrous connections; review design for conformance to all local, federal and state codes; inspect, verify and approve all personnel safety equipment such as but not limited to: hard hats. Safety harnesses, life lines, fall protection devices, advise the contractor in writing to remove any item found to be in non compliance to safety standards; advise the ACO in writing immediately of any non conforming items; perform a pre-construction inspection of the tower and document thru pictures any existing damage; review any structural loading imposed on the tower with new equipment; review paint color and compliance to existing paint. Check all damaged areas for correct painting, coats and thickness. Work also includes shop drawing review in the following manner: be responsible to research and pull out and review any previous construction shop drawings that were accomplished on this project; [maximum turn around time will be 5 working days for each submittal submission; formally submit to acquisition coordinator all approved/disapproved submittals]; site investigation-minimum of five (5) 8 hour site visits or ten (10) four hour site visits will be required during the construction phase. All site visits shall be coordinated between the a/e and by the administrative contacting officer. A/E to check in with the ACO office upon arrival and departure from the site. Anticipated site visits shall be but not limited to: a. pre-construction meeting b. 2-3 site visits during the construction period more if conditions warrant. Verify products/materials utilized comply with specified products and installation of equipment/materials is in compliance to contract documents. c. Final inspection preparation of punch list and completion of work-inspect for completeness-check for damage to tower and specify corrective action, site cleanup. Inspect work for compliance to contract documents. Prepare punch list progress. d. Site visit 30 days after issuance of punch list to record punch list progress. e. Submit by fax or hand deliver a professionally prepared field investigation report to acquisition coordinator (ACO) maximum of 24 hours following each site visit. As-built a complete as-built drawing on standard size Mylar drawing detailing actual installation of all new work signed and sealed by licensed professional engineer. A/E shall provide and utilize their own safety harnesses; lifelines, hardhats etc. submit documentation to the ACO that their safety equipment has been inspected and is safe to use. To be regarded as responsive for this announcement all prospective qualifying professional engineers [PE]: MUST be familiar with water tower designs, properly trained, have climbing experience and training, and be extremely familiar with cell phone antennae installations on water towers; PE MUST be able to review proposed cell phone antennae drawings and installation and inspect the installations for the Veterans Administration; PE MUST also have a MINIMUM of three (3) years prior experience climbing water towers. PE MUST be within a 50-mile radius of VAMC, Northport, NY. All work shall be performed in close coordination with building engineers. All work shall comply with the National Electric Code, Uniform Building Code, National Fire Protection Code and Occupational, Safety, Health Administration (OSHA) regulations. North American Industry Classification System (NAICS) Code is 541310. SF254?s and SF255?s are due in this office by COB 4/30/04.
