Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. The solicitation is issued as a Request for Quotation (RFQ). The solicitation number is RFQ 247-0317-04. This procurement is being conducted under Simplified Acquisition Procedures in accordance with FAR Part 13. The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-22. The VA intends to award a firm-fix priced contract for Health Care Resources in accordance with Public Law 104-262 and 38 U.S.C. 8153. Contractor shall provide a Psychiatrist to work at the Greenville Outpatient Clinic, Greenville, South Carolina, for treatment of eligible veterans and other eligible beneficiaries. PRICE IS AN ALL-INCLUSIVE RATE OF PER DIEM, TRAVEL. NOTE: FSS Contractors shall quote prices under their FSS contract and state their FSS Contract Number. Terms and Conditions included in this solicitation to not apply to any delivery order that may be awarded against an FSS contract resulting from this RFQ. CLIN1: Base Period - Date of Award through six-month period Description: The contractor shall provide one (1) Psychiatrist to work as described in the statement of work for an estimated 1000 hours. CLIN 2: Option Period 1 - Description - The contractor shall provide one (1) Psychiatrist to work as described in the statement of work for an estimated 1000 hours. Period of performance will be from date of award through a 6 month period, with one (1) additional six-month option periods that may be exercised by the Government. STATEMENT OF WORK: 1. GENERAL: The contractor shall provide the equivalent of 1.0 FTE psychiatrist to perform outpatient services to VA beneficiaries at the Greenville Outpatient Clinic (GOPC), Greenville, South Carolina. The definition of 1.0 FTE is a contract employee working 8 hours per day, five days per week. All services to be performed by the contract psychiatrist will be performed in accordance with VA policies and procedures and the regulations of the medical staff by-laws of the Dorn VA Medical Center, Columbia, South Carolina. The services to be performed and the contract psychiatrist will be under the direction of the Director, Mental Health Care Service Line (in Columbia, SC), and the on-site Manager at GOPC, Greenville, SC. 2.HOURS OF WORK: Contract Psychiatrist services will provide five (5) days per week at the GOPC, 8:00 a.m. through 4:30 p.m. excluding federal holidays. (b)The contract psychiatrist will work weekdays excluding Federal Holidays which include New Year?s Day, Martin Luther King?s Birthday, President?s Day, Memorial Day, Fourth of July, Labor Day, Columbus Day, Veteran?s Day, Thanksgiving Day, Christmas Day or any other day declared a federal holiday by the President of the United States. 3. PRIVACY AND SECURITY: Personnel Clearances and Privacy Act Considerations. Contract psychiatrist(s) shall be required to observe the requirements imposed on sensitive data by law, Federal regulations, VA statutes and policy, and associated requirements to ensure appropriate screening of all personnel. Because of the inherent sensitivity of the data maintained by the medical center, the level of security clearances required of the Contract psychiatrist will reflect the same level of security clearance carried by similar VA personnel for the function under contract. The contractor shall meet the above restrictions and assure that confidential and proprietary Services to be performed, shall include, but not limited to mental status, psychiatric evaluations, psychotherapy, and medication management according to the Medical Staff Bylaws. The incumbent will have all the responsibilities under Medical Staff Bylaws with the exception of committee assignments. The services to be performed by the contractor shall be performed within the VA policies and procedures and the regulations and bylaws of the medical staff of the VA facility. (a) Attendance and hours worked will be monitored daily through a log book requiring sign in and sign out. Payment will be authorized for any additional hours worked and must be pre-approved by the Manager, GOPC or his designee. (b) It will be expected that the psychiatrist will attend staff meetings at GOPC. The psychiatrist will contribute to interdisciplinary treatment planning for the patients, in accordance with JCAHO requirements and Service Operating Policies. (c)Privileges must be granted by the Professional Standards Board before any services can be provided under this contract. Administrative duties to be performed do not include the supervision of or performance appraisals for VA employees. (d) Administrative duties include, but are not limited to record keeping, reporting, and typing information into the electronic computerized record. 4. QUALIFICATIONS: Personnel assigned by the contractor to perform the services covered by the contract shall be licensed in a State, Territory, or commonwealth of the United States or the District of Columbia. The qualifications of such personnel shall also be subject to the review by the VA Chief of Staff and approval by the VA Facility Director. Personnel assigned to perform services covered by this contract must be Board Certified or Board eligible. 5. SOLICITATION: This solicitation adheres to the format defined in FAR 12.303 for the acquisition of commercial products and services. 6. WORK STATEMENT: (A) The Outpatient services performed will be: initial assessment and treatment of new patients, including those in crisis; the management of outpatients, including the medication, the appropriate use of psychotherapeutic interventions, and other duties that are assigned that are consistent with his/her credentials and privileging. (B) The VA formulary will be used and will be strictly adhered to when prescribing medications. (C) The Contract Psychiatrist will carry a workload averaging 9-14 patients each clinic day, seeing more some days and less other days. The number of patients treated each month will be a minimum average of 10 patients each day. This number will vary depending on a number of factors such as weather, holidays, political events, wars or anything that can affect the functional processes of the veteran bringing him/her to GOPC. The psychiatrist will complete commitment papers (for involuntary commitments). The incumbent will complete all required electronic clinical reminders as part of adherence to existing performance measures. All services provided to patients will require electronic documentation in the CPRS System. Typing will be required. (D) The Psychiatrist will be assigned to outpatients in the clinic from 8:00 am to 4:30 p.m., five days per week. (E) The Psychiatrist must: 1). Have an unrestricted license in a US state, territory or US commonwealth; 2). Speak English proficiently; 3). Provide evidence of demonstrated current clinical competence to perform appropriate outpatient care services; 4). Be a US Citizen; 5). Board Certified preferred, but he/she will be considered if Board Eligible 6). Have a professional degree and education applicable to medical staff members, e.g., MD, DO; 7). Provide evidence of current professional liability insurance; 8). Evidence of current privileges (if applicable); 9). Names of most current employer(s) and three peer reference contacts that can comment through personal observation of the psychiatrist?s work over the last 12 months; Failure to provide current references as defined here will result in non-consideration of that psychiatrist. 10). Current copy of BCLS certifications; 11). Be a team player; 12) Must be computer literate, as a computerized patient medical record is utilized. (F) Provider assigned by the contractor to perform the services stated in this contract will be credentialed and receive privileges by the VA. (G) The services to be performed by the Contractor will be performed within the VA policies and procedures and the regulations of the Medical Staff Bylaws of the Dorn VA Medical Center, Columbia, South Carolina. (H) Contractor?s employee must be present at the VA facility during working hours and must be performing the required services identified in the Work Statement or the cost will be decreased accordingly during each billing cycle. Likewise, failure to complete required clinical reminders or to place progress notes and complete encounter forms will result in an equivalent reduction in payment during a given pay cycle. The payments for holiday time (except for work performed in the case of an emergency) and for any leave, including sick leave or vacation time, are the responsibility of the contractor. (I) The contractor will be required to use the hospital computer system, to include but not limited to, checking electronic reports on a daily basis for accuracy and completeness, writing orders (i.e., Lab, consults and admission) and approving patient results via electronic signature. (J) The contractor will be required to stay current on reading e-mail as this is the means of communicating with staff. (K) The services specified herein may be changed by written modification. The modification will be prepared by the VA Contracting Officer prior to becoming effective, shall be approved by the VA Undersecretary for Health or his/her designee. (L) The parties agree that such personnel shall not be considered VA employees for any purpose and shall be considered employees of the Contractor. (M) The Contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the Contractor shall provide the following for those personnel: 1). Worker?s compensation 2). Professional liability insurance 3). Health examinations 4). Income tax withholding 5). Social Security payments (N) The services covered by this contract shall be furnished by the Contractor during work hours as defined herein. The Contractor will not be required, except in case of an emergency, to furnish such services on a national holiday or during non-working hours as described below. The following terms have the following meanings: 1). Work hours: 8 hours per day between 8:00 a.m. and 4:30 p.m.; five days per week. 2). Overtime (pre-approved) past 4:30 p.m. for patient care purposes. 3). Non-work hours: 16 hours per day between 4:30 p.m.; Monday through Friday, five days per week, and 24 hours per day, on Saturday and Sunday, two days per week. The NAICS Code is 621112 and the size standard is $8.5 Million.
