Description
SOURCE SELECTION FOR INDIAN HEALTH SERVICE, EMERGENCY DEPARTMENT REMODEL, HOPI HEALTH CARE CENTER, POLACCA, ARIZON Phase I of this solicitation will be a competitive formal Source Selection process in accordance with Part 15 of the Federal Acquisition Regulation (FAR) to ensure selection of the source evidencing the best overall capability to perform the work in a manner most advantageous to the Government, as determined by evaluation of proposals in accordance with the established criteria. Oral interviews may be held at the option of the Government. The most highly qualified firms selected will be asked to participate in Phase II of this procurement and to prepare a detailed cost proposal for further consideration by the Government. DESCRIPTION OF WORK: The Contractor shall provide all labor, material, supplies, transportation, equipment supervision and all other necessary items for a complete and useable project, performing all work in a workman like manner in complete compliance with all plans, drawings, and specifications issued and approved for the remodeling of the Emergency Department and associated work at the Hopi Health Care Center. The work consists of demolition, relocation of selected walls and door openings, new finishes to match existing work, modifications to mechanical and electrical work to accommodate interior changes, new automatic entrance doors, and addition of masonry veneer in corridors and modifications and additions to existing parking areas. The estimated project size is between $ 100,000.00 and $500,000.00. BID BONDS, PAYMENT AND PERFORMANCE BONDS ARE REQUIRED SET-ASIDE NOTICE: This procurement is set aside under the Small Business Competitiveness Demonstration Program, FAR Part 19, as unrestricted competition. Additionally, in accordance with Public Law 100-656, this procurement is a 51% Buy Indian Set Aside. The proposed contract listed herein is Set Aside under the Buy Indian Act for 51 percent Indian owned, controlled and operated firms. In accordance with the Indian Health Service Acquisition Regulation, Paragraph 370.503(E), not more than 50 percent of the work to be performed under a prime contract awarded pursuant to the Buy Indian Act, shall be subcontracted to other than Indian firms. For this purpose, work to be performed does not include the provision of materials, supplies or equipment. Contractors shall indicate whether they are 51 percent Indian owned, controlled and operated firm. In order for Indian firms to prove they can meet the qualification requirements of the Buy-Indian Set Aside restriction, Offerors who are currently certified by the Indian Health Service or the Bureau of Indian Affairs as an Indian Firm, need to submit certification, Tribal Affiliation and Degree of Indian Blood with their offer. Offerors who are not currently certified by IHS or BIA are required to submit proof of ownership and eligibility for Indian preference under the Buy Indian Act. Joint Ventures will be considered as long as the Indian firm is the managing entity. A copy of the joint venture agreement must be submitted with offer. MANDATORY REGISTRATION REQUIREMENT: FAR 52.204-7 Central Contractor Registration: Any and all firms or persons wishing to respond to this solicitation must be registered in the ?Central Contractor Registration (CCR) database? which is the primary Government repository for Contractor information required for the conduct of business with the Government. Offerors and Contractors may obtain information on registration and annual confirmation requirements via the internet at http://www.ccr.gov or by calling 1-888-227-2423, or 269-961-5757. By submission of an offer, the offeror acknowledges the requirement that prospective awardees shall be registered in the CCR database prior to award, during performance, and through final payment of any contract. PHASE 1: Competitive formal Source Selection process in accordance with Part 15 of the Federal Acquisition Regulation (FAR) will be used to ensure selection of the source evidencing the best overall capability to perform the work in a manner most advantageous to the Government, as determined by evaluation of proposals in accordance with the established criteria. Prime Contractors assembling a team are cautioned that that the evaluation will be based on the team, not just the prime firm. The Offerors must demonstrate its experience and professional ability in the following areas: (1) EXPERIENCE AS A PRIME CONTRACTOR WITH PROJECTS OF SIMILAR SIZE, SCOPE, AND COMPLEXITY: Offeror must show construction experience on similar projects of similar size and scope completed in a hospital environment which is fully operational. Elaborate on projects that have been completed in the last 3 years. Provide reference that may be contacted. (2) PROFESSIONAL QUALIFICATIONS AND EXPERIENCE OF THE PROPOSED STAFF FOR THIS PROJECT: Professional qualifications of project managers, superintendents, estimator, quality control, and construction inspectors. (3) MANAGEMENT PLAN: Proposed project approach, construction method to accomplish contract requirements including teaming planning between contractor/sub-contractors, the designer of record and Government representatives. Cost management; Critical path scheduling; Value engineering; Fast track experience must also be presented for consideration. (4) QUALITY CONTROL/QUALITY ASSURANCE/ SAFETY EXPERIENCE DURING CONSTRUCTION PHASE: Description of specific safety and quality control initiatives and plans for this project. (5) PAST PERFORMANCE WITH GOVERNMENT AND PRIVATE INDUSTRY CONTRACTS OF SIMILAR SIZE, SCOPE, AND COMPLEXITY: Past performance on contracts with Government entities including Native American organizations, tribal governments, federal agencies, i.e., Indian Health Service and Bureau of Indian Affairs. In accordance with FAR 15.305(a)(2)(ii), the Offerors is required to identify past or current contracts (including Federal, State, and local government and private enterprise) for efforts similar to the current Government requirements in this solicitation. The Government shall consider this information as well as information obtained from other sources when evaluating the Offerors past performance. The Offerors is reminded that the Contracting Officer shall determine the relevance of similar past performance information. The Offerors shall limit this information to not more than 10 contracts but no less than 3 similar contracts, performed within the last 3 years. Failure of an Offeror to provide a minimum of 5 relevant references or the inability of the Government to reach at least 3 references, after making a reasonable effort to do so, may result in the Offeror not being rated on the past performance factor. An Offerors who has no relevant experience will be given the mid-point rating of the maximum possible score allowed for past performance. Past Performance information must include the following information and be submitted in the following format and order: 1. Contract Number; 2. Contract Award Date; 3. Contract Type (e.g., fixed price, cost reimbursable, etc.); 4. Contract Term (e.g., one year, base year and option years); 5. Contract Dollar Value; 6: Name of Procuring Organization; 7. Address of Procuring Organizations; 8. Procuring Organization Contracting Officer?s Name and Phone Number; 9. Procuring Organization Technical or Program Manager?s Name and Phone Number; 10. Indicate whether the contract was a prime contract or whether the contract was a subcontract to a prime contract. If a prime contract, insert ?Prime,? if a subcontract, insert subcontractor. 11. Provide information on problems encountered on the identified contract and the corrective actions taken. PHASE II The most highly qualified firms will be selected and will be asked to participate in Phase II of this project. The firms selected for this phase will be provided plans and specifications for the project and requested to submit a cost proposal. PRICE EVALUATION FACTORS: Offerors are advised that an award may be made without discussion, exchanges or any contact concerning the proposals received; therefore, Offerors should present their best and final price at the time of submission. Offerors should not assume that they will be contacted or afforded an opportunity to qualify, discuss, or revise their proposals. Proposals will be evaluated on lump sum price for the base item that is most advantageous to the Government. Option items will be evaluated as to reasonableness of price and may be added at any time during the term of the contract as funds become available. The Government reserves the right to reject any or all proposals at any time prior to award; to negotiate with any or all Offerors on any prices, to award a contract to other than the offeror submitting the lowest total price; to award to the offeror submitting the proposal determined by the Government to be the most advantageous to the Government. The final selection will be determined based on both the technical proposal and the cost proposal submitted by each firm SPECIAL NOTES: A PRE-PROPOSAL CONFERENCE WILL NOT BE HELD FOR PHASE I. OFFERORS MUST RESPOND TO THE REQUIRMENTS OF THIS SOLICITATION TO BE CONSIDERED. THERE ARE NO OTHER DOCUMENTS AVAILABE IN PHASE 1. Qualification packages are to be sent to: Mr. William E.
