Description

Synopsis: The Great Lakes Acquisition Center (GLAC) is soliciting quotations to provide VA Iron Mountain, Michigan with one (1) part-time Primary Care Physician, M.D. services for its beneficiaries in the upper peninsula of Michigan. Place of contract performance is the Menominee, Michigan Community Based Outpatient Clinic (CBOC). Anticipated period of performance is May 1, 2004 through April 30, 2005 with (4) option year renewal periods, at the Government?s discretion. Contract type of firm-fixed price. Cost per hour to be all-inclusive (all labor, supervision, transportation, per diem, insurance, etc.). Work hours include 16 regular work hours per week (8:00am ? 4:30pm) two days per week, 52 weeks per year. Anticipated award date is April 15, 2004. This solicitation is 100% set-aside for small business. Electronic version and hard copy of solicitation will be available on or about March 15, 2004. This solicitation is issued under the authority of Public Law 104-262 and 38 USC 8153. This procurement is being conducted under the Subpart 13.5 test program where simplified acquisition procedures apply and the conduct of the procurement will be in accordance with Parts 12, 13, or 15 of the FAR or some combination thereof. Requests for hard copy solicitation must be in writing to the GLAC Contact Point and accompanied by a check for $5.00 made out to the Department of Veterans Affairs.
