Description
The Armed Forces Retirement Home - Gulfport intends to solict proposals for physician services. The Contractor shall operate an on-site Ambulatory Care Outpatient Clinic for a minimum of 24 hours per week. This clinic is a shared resource with the government staff. AFRH-G has approximatley 590 residents. The majority of the clinic's patients (approximately 480) reside in the AFRH Tower and are enriched/independent residents. Residents from the Assisted Living, Special Care, and Long-term Care may also utilize this clinic. The Contractor shall also provide ancillary duties. The time spent for ancillary duties will be approximately one (1) hour monthly. The physician(s) shall provide 12-20 hours a week to provide care for the ?inpatient? areas on the AFRH-G facility. The Contractor shall be responsible for providing the full range of medical care and shall be in compliance with JCAHO/regulatory standards for the Long Term Care Unit and Special Care residents and shall provide comprehensive professional, clinical and medical services in a geriatric environment. The Long Term Care Unit (LTCU) and the Special Care Unit have a patient load of approximately 15 - 24 residents in each unit. The physician shall also provide medical treatment for those residents in the Assisted Living Unit (ALU), who choose to use them as their primary care physicians, and shall assist in communication with other outside medical providers to ensure excellence in quality of care, as needed and appropriate. The Assisted Living Unit has 56 beds, with the potential for additional beds with programmatic changes. During hours of operation, the physician is responsible for attending to Tower, Assisted Living, Special Care, and Long Term Care residents for scheduled and ?walk-in? medical appointments. AFRH-G currently has 590 residents but can support up to 620. All residents have Medicare, Tricare or Champus and a supplementary medical insurance policy. At present time, Tricare requires that services be requested through the primary care physician in order for Tricare to be billed for services. The Contractor shall directly bill the resident's insurance company for reimbursement of services. Resident?s insurance companies shall be responsible for payment of services. The Government is not responsible for payment and shall not be billed for these services, including co-payments. Insurance information and informed consent will be provided to the Contractor by the residents of the AFRH-G at the time services are rendered. The Contractor is responsible for ensuring the residents fully understand that their private insurance will be billed for services. In the case of a resident who is under a conservatorship, the AFRH-G will be responsible for providing insurance information and in assisting with Informed Consent, where applicable, before treatment or services are provided by the Contractor. The Contractor shall do their own billing to insurance and/or Medicare as prescribed by law, following all legal and ethical guidelines. Their office and clinic staff will be provided by the Contractor. Insurance information and informed consent will be provided to the Contractor by the residents of the AFRH-Gulfport at the time services are rendered. The Contractor is responsible for ensuring the residents fully understand that their private insurance will be billed for services, or the POA is contacted for billing issues. The AFRH-G is not responsible for delinquent bills accrued by residents or their POAs/conservators. The Contractor shall be responsible for providing all medical treatment following all prescribed dictates of their professional and licensure codes as physician(s) in good standing and licensed and credentialed as governed by the physician(s) regulators. The Contractor and all employees providing services to AFRH-G residents will be credentialed in accordance with Federal and State law and AFRH-G policies, procedures, and dictated by JCAHO regulations, and any other governing or required regulations adhered to by the AFRH-G. The Contractor shall adhere to all requirements, regulations, and ethical practices, standards and licensing requirements for physician(s) in good standing under their regulatory agency or agencies. The physician(s) will agree to have a special clearance background check performed which will then enable physician(s)to use Keesler Air Force CHCS computer system to input medication orders and gather medication profiles on residents; utilize other services directly; and potentially enter laboratory requests. The Contractor shall fulfill all necessary requirements for credentialing under the rules and regulations of the federal government and maintain licensure as appropriate for a physician of good standing for the state of Mississippi. Their competency information, CEUs, and annual PPDs will be kept up to date as required by contracting through the government. Any changes or legal or liaility issues will be communicated to the Healthcare Services Administrator or Credentialing Official of AFRH-G. The Contractor shall provide quality and appropriate documentation through handwritten or transcribed (by their own means) progress notes as appropriate and meeting all regulatory standards regarding all contact with residents as dictated by law, regulations, requirements, per their regulating agency, and in maintaining JCAHO requirements, and by AFRH-G policies and procedures. The laboratory and X-Ray needs of residents will be done through the Contractor unless otherwise specified by resident. All medical and business supplies needed for the care of the residents in the clinic area will be supplied by the Contractor.(e.g. Nitrogen for dermatology, in house medical supplies, etc.) The AFRH-G provides the clinic space, utilities, and attached physical assets needed to provide medical care for the resident population. (furniture, exam room equipment). Office space and exam areas in the clinic area will be used by non-Contractor personnel (i.e. Government and other contractor personnel). The Contractor shall update the medical charts in the shared clinic. The medical records remain the property of the AFRH-G, to include but not limited to Histories and physicals and Discharge summaries. The contract physicians shall follow JCAHO/regulatory guidelines and requirements in the care of and treatment of the LTCU, Special Care, and ALU residents. The Contract Physician(s) is responsible for ordering necessary laboratory, exams, and radiological procedures to meet examination and evaluatory requirements, and is responsible for assuring each physical examination performed is complete and that all medical problems are documented with a plan for proper evaluation, treatment, referral, or follow-up. The Contract Physician(s) is responsible for recognizing various medical pathologies and referring residents who require specialty care for diagnostic and treatment procedures or measures. The Contract physician(s) is responsible for providing a full treatment regimen involving the knowledge of new techniques or the use of prolonged or complicated procedures or advanced and delicate skills and to communicate effectively with staff and residents.
