- This is a combined synopsis/solicitation for a commercial item, prepared in accordance with the format in FAR Subpart 12.6, and supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Q uotes are requested; however, a written solicitation will not be issued. Solicitation number W911S8-04-T-0019 is applicable and is issued as a request for quotations. The solicitation and incorporated provisions and clauses are those in effect through Fe deral Acquisition Circular 2001-03. The requirement is small business set-aside with a NAICS Code of 423120, SIC Code 5013, and size standard of 100. Manufacturer is Wooten Metal, Inc. The description of the commercial items are as follows: Clin 0001: Low Boy Rack, 23 each; Clin 0002: High Boy Rack, 5 each; Clin 0003: HMMWV Net, 28 each; M-Gator Rear Basket Net, 5 each. State if FOB is Destination or Origin. If Origin, state shipping charges. Delivery is to be approximately 1-2 weeks after receipt of ord er to DOL Central Receiving Point, Fort Lewis, WA 98433-9500. Any award resulting from this request for quotations will be made to a responsive, responsible quoter using a SF1449 form. Quotes will be evaluated on a lowest price evaluation factor. Award w ill be made on an all or none basis. If quotes are provided for an equal item, specifications must be submitted. Provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. An addendum to this provision is applicable, in w hich the term offeror (whenever it appears) shall be replaced with the term quoter. Quoters are required to submit completed copies of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their quotes. FAR cla use 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and does not include any addenda. Far clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, applies to this acquisition. The following clauses within 52.212-5(b) are applicable to this acquisition: 52.222-3, 52.222-19, 52.222.21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-33. The following clause within DFARS clause 252.212-7001(b) is applicable to this acquisition: 252.225-7001.
