Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis and solicitation for the following commercial items, Item 1: 01-31 March 2004, 14 rooms x 31 nights. Within 10 miles of the Minnesota Air National Guard Base. Rooms will include a high standard of hygiene, private bath with full toilet and shower facilities, hot and cold water, clean linen and cleaning service (provided on a weekly basis). Electricity, microwave, refrigerator, coffee maker, ironing board/iron, and hair dryer, television set, telephone, alarm clock or wake up call service, and appropriate furniture (e.g. One King, Queen, Full, or Double bed; night stands, lamps, drapes/curtains on windows, dressers). Any additional charges (e.g., telephone calls, premium cable station charges, room service, laundry service, mini bar, damages to room, or any other hotel service charge) shall be billed to the guest residing in the room in which the charge is made. Collection of any additional charges incurred by ind ividuals will be the responsibility of the contractor. It is suggested that a credit card be provided upon check-in to cover any incidental costs. Item 2: 01-30 April 2004, 14 rooms x 30 nights. Rooms will include all items stated in line item 0001. Item 3: 01-31 May 2004, 14 rooms x 31 nights. Rooms will include all items stated in line item 0001. Item 4: 01-30 June 2004, 14 rooms x 30 nights. Rooms will include all items stated in line item 0001. Item 5: 01-31 July 2004, 14 rooms x 31 nights. Rooms will include all items stated in line item 0001. Item 6: 01-31 August 2004, 14 rooms x 31 nights. Rooms will include all items stated in line item 0001. Item 7: 01-30 September 2004, 14 rooms x 30 nights. Rooms will include all items stated in line item 0001. This solicitation is issued as a request for quote (RFQ) W912LM-04-T-0021. Submit written offers, oral offers will not be accepted. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). This solicitation do cument and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-17 and DFARS Change Notice 2003-1001. North American Industrial Classification Standard 721110, Size Standard, $6 million, applies to this procurem ent. FOB: Destination for delivery to SMSGT JEFF KNEPPER, 148 FW BASE SUPPLY, 4625 DEUCE AVENUE, DULUTH MN 55811. The following provisions and clauses apply to this acquisition: FAR 52.211-6 Brand Name or Equal. The provision at FAR 52.212-1, Instruction s to Offerors--Commercial Items. Quotes will be evaluated on price related factors only. The provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, and the offeror must include a completed copy of this provision with th eir proposal. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items.
