THIS IS A SOURCES SOUGHT NOTICE. FOR FURTHER INFORMATION CONTACT MR. VINCENT PACCHIANA CLINICAL ANALYST AT 706-787-2094. EMAIL ADDRESS: vincent.pacchiana@se.amedd.army.mil. This FEDERAL BUSINESS OPPORTUNITY announcement is a notice that TRICARE Regions 3, 4 & 15 that include all DOD Medical Treatment Facilities (MTFs) in the states of Alabama, Florida, Georgia, Mississippi, South Carolina, Tennessee, and Southcom are in the process of standardizing Exam Room Supplies as listed in the subject lin e above. Companies with current Distribution and Pricing Agreements (DAPAs) with the Defense Supply Center Philadelphia for these items are eligible to participate in the standardization process. Base company criteria for this product line includes but is not limited to the following: must carry a full product line, must provide customer support, in the form of a sales representative for each MTF within TRICARE Regions 3, 4 & 15, must provide clinical support, education programs, and utilization reports, must provide company return policy, must provide company distribution network, must be able to provide product for all MTFs within TRICARE Regions 3, 4 & 15, and must provide electronic and hard-copy of proposed pricing and competitive product cross-refere nce. Base product criteria for this product line includes but is not limited to the following: Cotton Tip Applicators- Available in a variety of sizes and lengths, plastic and wood shafts, both sterile and non-sterile. Sterile cotton tip applicators must be available in packages in of two. Tongue Depressors- Available in a variety of sizes, finish should be smooth and splinter-free, both sterile and non-sterile. Respondents must address all criteria listed above. Please contact Mr. Pacchiana prior to sen ding your response along with literature and representative product samples (enough to share with a 10 person Clinical Evaluation Team) of your company exam room supplies.
