- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.603, as supplemented with additional information included in this notice. This procurement is also issued under the authority of FAR 13.500. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation ??????W912DY-04-T-0006?????? is issued as a Request for Quotat ion (RFQ). This solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 97-27. NAICS CODE is 811219. This acquisition is 100% set-aside for Small Business. Small Business Size Standard is $6M. DESCRIPT ION: Contractor shall provide all services, including, but not limited to, personnel, material, labor, travel and lodging necessary to perform maintenance of fitness equipment, located at the U.S. Army Corps of Engineers, 4820 University Square, Huntsvill e, AL 35816-0001. Maintenance shall consist of: 1. Monthly cleaning and lubrication as needed on all of the exercise equipment except mats, balls, dumbbells, gloves, etc. 2. Semi-annual inspection of treadmills and all other electronically operated fitne ss equipment. This service includes opening the unit, cleaning, lubing, adjusting, and checking for worn parts and electrical safety inspection. Includes labor to replace, but not cost of, worn parts. Does not include labor to replace deck or walking be lt on treadmills. 3. Annual inspection of non-electrically operated equipment. Generally pulley/cable-driven machines. These will be inspected for wear of surfaces not visible to the monthly inspection. Includes inspecting springs, cables, pulleys, rol lers and bearing. Cleaning and lubrication of these surfaces. Includes labor to replace, but not cost of, worn parts. Existing equipment consists of Cybex, Smith Press w/fixed bar, (wts: 8-5lb; 6-2.5lb; 8-10lb; 6-25lb; 6-35lb; 8-45lb); Parabody Bench, t wo (2) ea; Flat Bench; Body Solid Bench Incline; Cybex 700 Upright Bike; Cybex 600R Bike; Stairmaster 4000PT; Techrix Climbmax, two (2) ea; Reebox BodyTree II; Cross Aerobics Stairmaster; Cybex Shoulder Press; Cybex AB Crunch; Cybex Back Extension; Precor C964 Treadmill, two (2) ea; Precor C944 Treadmill, two (2) ea; Cybex Dual Axis Row/Rear Delt; Cybex Fly; Concept II Rower; Cybex Chest Press; Cybex Leg Curl; Cybex Cable Column; Cybex Lat Pulldown; Cybex Modular Leg Press; Cybex Modular Assist Dip/Chin; UB S Body Bag Frame w/bag; Parallel Bars; Ross Stationary Bike; Step Platforms, nineteen (19) ea, with eighty (80) ea extensions. This will be a firm-fixed type contract with two option years with Basic Contract Period of Performance from 1 March 2004 throug h 28 February 2005; Option Year 1, 1 March 2005 through 28 February 2006; and Option Year 2, 1 March 2006 through 28 February 2007. Factors 1 , and 2, are equal in importance and Factor 3 is significantly less important than Factors 1 and 2. The non-price factors when combined are significantly more important than price.
