- Government Furnished Items 10.0 The designated COTR shall provide the Contractor with weekly group attendance rosters complete with client name; PDID number; space for client signature; and space for contractor certification. 10.1 At any point during the life of this contract, the Government may opt to provide the contractor with use of a facility adequate to perform the required services. This facility would be located within the District of Columbia. In this event, the Government will give the Contractor at least 30 days notice of the change in place of performance and will provide space adequate to perform the functions described herein. Period Of Performance The base period of performance for this contract shall be one year from the date Of contract award until September 30, 2004, with four (4) one-year option periods to be exercised at the Governments discretion in accordance with the clause 52.217-9 Option to Extend the Term of the Contract. Quality Standards 12.0 The designated COTR reserves the right to inspect and evaluate all services provided during the performance of this contract. The designated COTR may conduct surveillance of any task at anytime during the performance of this contract. All surveillance observations will be documented by the designated COTR. These surveillance observations may include, but are not limited to, observation of group sessions, review of client records and files, client interviews, and pre/post testing of clients. When an observation indicated defective performance, the Contracting Officer (CO), with the technical support of the COTR a Discrepancy Report may be issued to the Contractor. The Contractor shall address any Discrepancy Report in days of notification, indicating any corrective action taken to rectify the deficiency. The following Federal Acquisition Regulation provisions and clauses apply to this acquisition: The provision at 52.212-1, Instruction to Offerors-Commercial and 52.212-4, Contract Terms and Conditions-Commercial Items, apply to this acquisition. Offerors shall include a completed copy of the provision at 52.212-3, offeror representations and certifications-commercial items, with its offer. Delivery orders may be issued for up to one (1) year from the date of award plus a six month option that may be exercised; 52.216-19 Order Limitations (minimum 75 sessions, maximum 576 sessions); 52.216-21 Requirements; 52.217-8 Option to Extend Services; 52.217-9 Option To Extend The Term Of The Contract-Services; 52.219-6 Notice of Total Small Business Set-Aside, 52.224-1 Privacy Act Notification; 52.224-2 Privacy Act; 52.212-5 Contract terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items applies to this acquisition and the following FAR clauses cited in the clause are applicable to this acquisition: 52.219-8 Utilization of Small Business Concerns; 52.222-21 Prohibition of Segregated Facilities, 52.222-26 Equal Opportunity; 52.222-35-Affirmative Action for Disabled Veterans and Veterans of the Vietnam Ear; 52.222-36 Affirmative Action for Workers and Disabilities; 52.222-37 Employment Reports on Disabled Veterans and Veterans of the Vietnam Era; 52.232-18 Availability of Funds; 52-232-19 Availability of Funds for the Next Fiscal Year, 52.232-33 Payment by Electronic Funds Transfer Central Contract Registration, 52.233-2 Service of Protest, 52.237-7, 52.212-2 Evaluation--Commercial Items applies to this acquisition. The Government will make awards for the supplies or services offered in response to this solicitation that meet the definition of a ?commercial item? in FAR 52.202-1. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Technical Capability of the services offered to meet the Government requirement; (2) Price; and (3) Past performance. Technical and Past Performance, when combined, is more important than price. Past performance information shall include a list of three (3) of the most recent and relevant past and present contracts performed for Federal agencies and commercial customers within the last five years. Furnish the following information for each reference: (1) Company name, (2) Service provided, (3) Contracting Officer Name and Phone Number, (4) Contract Number, (5) Contract Dollar Amount, (6) Period of Performance. Award may be made with or without discussions. One original and three copies of the technical and cost proposal shall be submitted no later than 2:00 p.m. local time on February 6, 2004 to Attn: Darren Brown, D.C.
