Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR, Subpart 12.12 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ) number 688-25-04. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular number 2001-19. The NAIC code for this solicitation is 339116. The small business size standard is 500 employees. This solicitation is a small business set-aside. The contractor shall fully fabricate FLEXIBLE REMOVABLE PARTIAL DENTURES and LUCITONE 199 DENTURES ready for delivery to the patient. The SCOPE OF WORK FOR THE BASE YEAR shall be listed and priced as follows: 1. FLEXIBLE REMOVABLE PARTIAL DENTURES - CONTRACTOR SHALL PROVIDE FULL FABRICATION OF FLEXIBLE REMOVABLE PARTIAL DENTURES. (1) MASTER/OPPOSING CASTS WILL BE ARTICULATED BY VAMC?s CENTRAL DENTAL LAB (CDL). (2) A DUPLICATE OF THE BEADED MASTER CAST WILL BE PROVIDED BY CDL. (3) USE DENTURE TEETH AND FLEXIBLE RESIN SYSTEM RESIN PROVIDED BY CDL. (4) CREATE ADEQUATE RETENTION (DIATORICS) TO PREVENT DENTURE TEETH FROM LOOSENING OR DETACHING FROM THE FELXIBLE BASE. (5) RETURN THE PROTHESIS ON THE ORINGINAL, UNDAMAGED MASTER CAST EXHIBITING QUALITY ASSURED FIT, RETENTION, OCCLUSION, AND ESTHETICS (6) CASEWORK WILL BE COMPLETED IN A MANNER, AND RETURNED TO OUR CENTRAL DENTAL LABORATORY NO LATER THAN TEN WORK DAYS FROM DATE OF PICKUP. (7) CONTRACTOR WILL PROVIDE PICK-UP AND DELIVERY SERVICE FOR ALL CASEWORK FREE OF CHARGE. 2. LUCITONE 199 DENTURES - CONTRACTOR SHALL PROVIDE BACKUP SUPPORT DURING PEAK WORKLOAD AS NEEDED FOR SETUP AND/OR PROCESSING OF LUCITONE 199 DENTURES (1) MASTER CASTS WILL BE ARTICULATED BY CDL. (2) USE DENTURE TEETH PROVIDED BY CDL. (3) PROCESS ALL DENTURES WITH LUCITONE 199 (SHADE "LIGHT") (4) TRAIL WAX SET-UP AND/OR WAXUP-PROCESSING WILL INCLUDE ESTHETIC AND ANATOMIC MUCOSAL/GINGIVAL CONTOURS, AS WELL AS BILATERALLY STABLE OCCLUSION. (5) CASEWORK WILL BE COMPLETED IN A SATISFACTORY MANNER AND RETURNED TO CDL FOR ARRIVAL NO LATER THAN SEVEN (7) WORKDAYS FROM THE DATE OF PICKUP. (6) CONTRACTOR WILL PROVIDE PICK-UP AND DELIVERY SERVICE FOR ALL CASEWORK FREE OF CHARGE. The SCHEDULE OF PRICES 1. Flexible Removable Partial Dentures 190 cases @ $__________per for a total of $___________ 2. Lucitone 199 Dentures 100 cases @ $__________per for a total of $___________ Please include in offer package a completed Standard Form 1449 that can be found at www.gsa.gov/forms/farnum, required provisions, and a schedule of prices for the year. Acceptance time will be 60 days from offer due date.
