- Description
- (i) This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation FQ4877-04-Q-0135 is being issued as a request for quotes. (iii) This solicitation incorporates clauses and provisions in effect through the Federal Acquisition Circular 01-07. (iv) This requirement is being synopsized as a small business set-aside under NAICS Code 562910, small business size standard of $12,000,000.00 (v) Please quote to perform the service as listed: Provide all labor, equipment and materials necessary to perform the following project: Tear-off roof in four (4) separate phases of 4,000 square feet each to allow roofers adequate time to complete roofing operations between tear-off and re-installations. Removal of roofing includes asbestos abatement, removal, and disposal. Contractor is responsible for all personal air monitoring equipment. All areas will be set up in accordance with NESHAP Regulations. Contractor must comply with all OSHA and Environmental Regulations. The contractor will need to measure exact dimensions. The contractor shall remove debris and dispose of them off-base at approved landfill. (vi) N/A (vii) Place of performance: Community Center, Bldg 4201, on Davis-Monthan AFB, AZ 85707. Work shall begin not earlier than 2 Feb 2004, with required completion by 31 Mar 2004. (viii) Provision 52.212-1, Instructions to Offerors-Commercial applies to this acquisition. (ix) Provision 52-212-2, Evaluation-Commercial Items applies to this acquisition; quote shall be evaluated on price. (x) Offerors are required to submit a completed copy of FAR 52.212-3 Offeror Representations and Certifications-Commercial Items (http://www.arnet.gov/far/loadmainre.html)with their offer. (xi) Clause 52.212-4, Contract Terms and Conditions-Commercial Items applies to this acquisition. (xii) Clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders-Commercial Items applies to this acquisition. (xiii) N/A. (xiv) N/A. (xv) N/A. (xvi) A mandatory pre-offer site visit shall commence at 09:30 a.m. local time, 8 January 2004 at the Community Center of Davis-Monthan AFB. To ensure base access, please meet contact POC not later than 8 September 3:00 p.m. to advise of your intent to attend the Pre-Offer Site Visit. Meeting point for actual Site Visit will be Security Forces Pass & Registration Trailer, located outside the main entrance gate to DMAFB (off of Craycroft Road), NLT 0915 on 8 January 2004. Completed quotes shall be due to the 355th Contracting Squadron, 3180 S. Craycroft Rd. Davis-Monthan AFB, AZ 85707, by 9 Jan 2004, 09:00 a.m. local time.
