- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are b eing requested and a written solicitation will not be issued. This solicitation, identified as W52H09-04-D-0131, will be issued through ASFI/FedBizOpps as will any amendments threrto. This solicitation is issued as a Request for Quotation (RFQ). The Sol icitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-16, effective 1 October 2003. This requirement for 35 each Rescue System, Medevac Litter Black, 402-B. There is no NSN. Inspection a nd Acceptance is at Origin. Deliveries are F.O.B Destination. Deliveries are within 90 days after receipt of award. CLIN 0001AA is for a quantity of 35. Delivery destinations and quantities are as follows:3 ea Birmingham, AL;3ea Ft. Carson, CO; 2ea Pan ama City, FL; 3ea Ft. Campbell, KY; 6ea Ft. Bragg, NC; 3ea Draper, UT; 10ea Ft. Eustice, VA; 2ea Ft. Lewis, WA; 1ea Okinawa, Japan; 2ea Schofield Barracks, HI. The following provisions apply to this acquisition: FAR 52.212-1 Instructions to Offerors-Comme rcial Items (OCT 2003); FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (JUN 2003)(a copy of this provision shall be completed and submitted with the offer); FAR 52.212-4, Contract Terms and Conditions-Commercial Item (OCT 2003); and FAR 52.212-5, Contract Terms and Conditions Required to implement Statutes or Executive Orders-Commercial Items (OCT 2003), along with the additional FAR clauses cited under paragraph (b) 15, 16, 17, 18, 19, 21, 29. This solicitation is restricted to Lifesaving Systems Corporation, 220 Elsberry Road, Apollo Beach, FL 33572-2289, offers from concerns other than the above are not solicited. Published for subcontracting purposes only. The proposed contract action is for supplies or services for which th e Government intends to solicit and negotiate with only one source under the authority of FAR 6.302-1. Interested persons may identify their interest and capability to respond to the requirements or submit proposals. This notice of intent is not a reques t for competitive proposals. However, the Government will consider all proposals received within fifteen days after the date of publication of this synopsis. A determination by the Government not to compete with this proposed contract based upon response s to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. Response is due by 19 December 2003, at 4:30 pm local time.
