- This announcement constitutes a Sources Sought synopsis seeking potential contractors to provide an investigative study of close telephone case management monitoring of patients with a diagnosis of heart failure (HF) to beneficiaries of the South Texas Veterans Health Care System. This service is to be provided by specially educated registered nurses to assess and teach patients about HF and quickly reversed acute clinical symptoms in a home setting. To include but not limited to training concerning daily weights, daily blood pressures, medications and their side effects, and customized diets including exercise and salt restrictions, for the patient's individual diagnosis. Contractor will be responsible for immediately completing documentation of each individual encounters, patient interviews, and all pertinent patient related information. Service must be available 24/7, it is anticipated both English and Spanish verbal and written teaching tools be made available to the patient. An estimated 150 patients will be involved in study, potential patients will be provided to vendor who will screen the veteran patients for willingness to participate in the program. In response to this Request for Information (RFI) at a minimum interested businesses shall provide the Government with the following information (1) Name of Company-include a contact person's name, telephone number, and email address; (2) Company Address; (3) Company Size (please specify large, small, hub-zone, 8(a)); (4) Specify whether the services identified above are of a type offered and sold competitively in substantial quantities in the commercial marketplace based on established catalog or market prices and whether these services are exclusively provided to the Government; (5) Commercial policies and practices as it relates to the following areas; minimum qualifications of the candidates; candidate rejection/acceptation policy, professional liability insurance limits, billing and payment terms and other standard commercial contract terms and conditions that would apply (6) Describe experience and past performance with similar types of requirements of similar scope and magnitude and whether they were performance-based - include the contract number; Government agency name and address; contract funded amount; (7) Identify existing or potential pricing structure in support of this type of work; and (8) Identify qualifications of technical and management personnel. ***** THIS SYNOPSIS IS FOR INFORMATION AND PLANNING PURPOSES ONLY AND IS NOT A COMMITMENT BY THE GOVERNMENT.
