- NA (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals ar e being requested and a written solicitation will not be issued. (ii) This request for quotation (RFQ) number is W912CZ-04-T-0004. (iii) This RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) 200 1-15. (iv) This acquisition is 100 percent small business set aside, in regards to size standards 1,000 employees. The applicable NAICS is 325188 (v) This RFQ contains one (1) line item. Requirement for Potassium Permanganates. QTY, 475 (55 lbs), & Sodium Carbonate for water treatment plant, QTY 2,000 (50 lbs) A. Contractor shall supply the chemicals that must meet AWWA standards for water treatment. B. Chemicals must be supplied in maximum 55 pound containers. C. Minimum Potassium Permanganate needed for one year 416 containers or 22,880 pounds. Maximum needed is 475 container or 26,125 pounds. D. Minimum Sodium Carbonate needed for one year is 1,500 containers or 75,000 pounds. Maximum needed is 2,000 containers or 100,000 pounds; an MSDS must ac company each delivery. E) All delivery under this contract will occur between 0800 and 1400 hours Monday thru Friday, except Holidays. F) Persons authorized to place calls under this contract will be provided in the award. (vi) Delivery location, Accepta nce will be Government FOB destination to Bldg 3565, Fort Wainwright, Alaska 99703. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition, along with the following addenda. (ix) Offers will be evaluated using the following evaluation factors technical acceptable, price and delivery time. The contract will be awarded to the offeror that provides the best value to the government. (x) All offers must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items (Alt I) (See attached) (xi) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items applies to this acquisition along with the following addenda: 52.219-1, Small Business Program Repr esentations (Apr 2002).
