- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and 13.106, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation Number F6015131810100 is issued as a request for quote (RFQ) and is unrestricted. The resulting contract will be Firm Fixed Price. Period of Performance will be 1 Oct 03 ? 30 Sep 04 Base Year, with (1) Option Year. Award date is expected to be not later than 1 Oct 2003. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-12. The NAICS code assigned to this project is 813110 and the size standard is $6,000,000.00. The requirement is for the 11th Wing/Operations Group HC-A, Washington, DC. The contract line item is: CLIN 0001 ? Roman Catholic Priest, Quantity 1. STATEMENT OF WORK ROMAN CATHOLIC PRIEST A. Description of Services Contractor shall provide all tools, labor, equipment, and materials to perform the following service. Contractor shall perform all services in accordance with the following Statement of Work (SOW). B. Service Delivery Summary (SDS) a. Individual is responsible for all Catholic funerals associated/scheduled with Arlington National Cemetery (ANC), including those scheduled at the Soldiers, Sailors, and Airmen?s Home. b. Conduct worship service/Mass in a chapel environment using Roman Catholic dogma. c. Perform military graveside internment service in conjunction with the USAF Honor Guard, USAF Band, and the ANC representatives, rendering appropriate military honors. d. Perform personalized graveside and/or chapel worship services for deceased spouses/dependents of military honors. e. Serve as focal point for the Air Force for Catholic funerals before, during, and after funeral service to include contacting the next-of-kin at least one week prior to the service in order to arrange for the religious portion of funeral service. f. Present American Flag to next-of-kin on behalf of the President of the United States at military honors funerals. g. Send written correspondence to the next-of-kin at least two days after service as part of ANC follow-up ministry. C. General Information a. Member will be prior Air Force due to the provisions of presenting the American Flag as outlined in the following regulations: *- Section 578 of Public Law 106-65, under section B paragraph 2: At least two members of the funeral honors detail for a veteran?s funeral shall be members of the armed forces, at least one of whom shall be a member of the armed force of which the veteran was a member. The remainder of the detail may consist of members of the armed forces or members of veteran?s organizations or other organizations approved for purposes of this section under regulations prescribed by the Secretary of Defense. Each member of the armed forces in the detail shall wear the uniform of the member?s armed force while serving in the detail. *- BAFBI 10-1, Section 1.7.3.: Arlington National Cemetery Air Force Chaplaincy acts as lead for HQ USAF/HC Chaplains Service on military funerals, honors and rites. Plans and executes religious funerals, committals, memorials and rites within ANC for present and former Air Force personnel and their dependents. Provides pastoral care to the next of kin (NOK) for funeral services. If additional care is required, family will be referred to their local family minister. Provides and coordinates escort, transportation and a waiting area for members of the USAF Arlington Committee. Provides other chaplain services (event invocations, benedictions, etc.) for 11 WG ceremonies, as resources allow. 1.7.3.1. Ensures religious services are conducted in accordance with the deceased and NOK?s faith and wishes. 1.7.3.2. Escorts visiting chaplains and clergy conducting religious services at ANC. 1.7.3.3. Presents the US flag to NOK, unless the NOK or higher headquarters has requested otherwise. b. Member will be familiar with military customs and courtesies as outlined in *AFPAM 36-2241V1, Military Customs and Courtesies (see item D). c. Member will be adhere to the standards of appearance as outlined in the following regulations: - *AFI 36-2903, Dress and Personal Appearance of Air Force Personnel - *AFPD 36-29, Military Standards NOTE: See item D for acquisition of regulations. d. Member will follow funeral guidelines as outlined in *training binder (see item D). D. Applicable Documents: a. All publications in this SOW identified with an asterisk (*) are government publications applicable to the contract. All are mandatory documents that will be adhered to. Should there be any changes to any of these publications following contract award, the individual will comply with all changes. These documents may be viewed at the Air Force Chaplains Office, Arlington National Cemetery, Arlington, VA 22211. E. Location of Duties: a. Duties will take place at - Arlington National Cemetery, Arlington, VA 22211 - Fort Myer Old Post Chapel, Ft Myer, VA 22211 - Fort Myer Memorial Chapel, Ft Myer, VA 22211 - Soldiers, Sailors, and Airman?s Home, Washington DC F. Substitutions: a. Contractor will arrange and compensate a qualified substitute prior to any absences for services. Substitute will be approved by the contracting officer. G. Qualifications/Experience: a. Will be a certified ordained Roman Catholic Priest in good standing with his religious superiors. b. Will have at least 4 years experience presiding over funerals. c. Will provide at least 2 written letters of qualification from religious superiors validating funeral experience. d. Will provide at least 2 written letters of recommendation from religious superiors. Inspection and acceptance shall be made at destination ?Bolling AFB, DC. The contractor shall extend to the Government full coverage of any standard commercial warranty normally offered in a similar commercial sale. Acceptance of the warranty does not waive the Government?s rights under the Inspection clause, nor does it limit the Government?s rights with regard to the other terms and conditions under this contract. In the event of a conflict the terms and conditions of the contract shall take precedence over the warranty. The warranty period shall begin upon final acceptance of the applicable deliverables listed in the schedule. FAR 52-212-1, Instructions to Offerors-Commercial is incorporated by reference and applies to this acquisition. Offerors shall provide back-up information supporting the price offered. This could take the form of the most recent catalog price, invoices from previous sales of same items, etc. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representation and Certifications ? Commercial Items, with its offer. The FAR Clause 52.212-4 Contract Terms and Conditions ? Commercial Items applies to this acquisition and will be incorporated into any resulting contract. The clauses at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders ? Commercial Items apply to this acquisition, specifically the following cited clauses and provisions are applicable: 52.222-21, Prohibition of Segregated Facilities; 52.222-22 Previous Contracts and Compliance Reports; 52.222-25 Affirmative Action Compliance; 52.222-26, Equal Opportunity; 52.222-35, Equal Opportunity for Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans; 52.222-36, Affirmative Action for Workers with Disabilities; 52.222-37, Employment Reports on Special Disabled Veterans, Veterans of the Vietnam era, and Other Eligible Veterans; 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration; and 52.223-11, Ozone Depleting Substances. The following DFAR clauses and provisions apply to this solicitation and are incorporated by reference: 252.212-7001, Contract Terms and Conditions Required to Implement Statutes Applicable to Defense Acquisitions of Commercial Items; 252.225.7001 Buy American Act and Balance of Payments Program; 252.225.7002 Qualifying Country Sources as Subcontractors; and 252.204-7004, Required Central Contractor Registration.
